*New Teachers and Special Education Teachers may earn more than the six (6) required days of in-service. In this case, the extra earned days may be applied toward the (2) Principal’s Discretionary days (with principal’s approval).

Certificated Personnel must earn (4) Days of In-Service Training. Please list all Discretionary and Professional Development events attended during the time period of May 26, 2017 and May 22, 2018. Late hires will have the opportunity to complete missed in-service during the school year. In the event that a scheduled In-Service is cancelled, affected personnel may substitute an alternate professional development training. However these alternate trainings must be pre-approved by the principal.It is the employee’s responsibility to maintain documentation and accurately report all required In-Service, Professional Development, and Discretionary dates of attendance.

Please “star”the title of any professional development activity which takes place after normal school hours (i.e. 3:30-6:30).

REQUIRED IN-SERVICE DAYS: No other in-service activities may be substituted for “Required In-Service Days”. All certificated personnel are expected to attend each required in-service day. Dates for scheduled in-services are subject to change if days are missed during the school year for inclement weather. The school calendar will be revised as required.

NOTE: Teachers work a 200 day contract which includes 180 instructional days, ten days of in-service, and ten vacation days. September 11, 2017, and January 2, 2018, are Staff Development Days from the stockpile. Stockpile days count as part of the 180 instructional days (students are present 178 days). Stockpile daysare not to be considered as in-service, principal’s discretionary or professional development days.

A full-time/full-year Teacher/Administrator/Truancy Officer should record ten (10) days of in-service training (6 hours = 1 day).

A full-time/full-year Instructional Assistant or Data Clerkshould record two (2) days of in-service training (6 hours = 1 day).

This is to certify that I have completed the number of in-service days as shown above and proof of attendance is on file. TCA 49-50-1402 through 49-50-1407 reviews the penalties for falsification of records. Falsification of records is grounds for dismissal.

This report is to be kept in a cumulative manner and should be maintained in the office of the Principal. At the close of the school year (or at the conclusion of the teacher’s employment should he/she leave during the school year), this form must be submitted to the Central Office. NOTE: If two or more teachers fill the same position during a school year, only a total of ten (10) days between or among the teachers involved may be reported and paid for the same teaching position. (Example: Maternity Leave – Teacher A reports 7 days; Teacher B reports 3 days = 10 days total). END OF YEAR DOCUMENTATION WILL BE MAINTAINED BY THE PRINCIPAL. THIS FORM MUST BE COMPLETED BY ALL PRINCIPALS, ASSISTANT PRINCIPALS, TEACHERS, INSTRUCTIONAL ASSISTANTS, DATA CLERKS AND TRUANCY OFFICERS. Revised Date
