2 / Apologies for Absence, Minutes of the Previous Meeting (13 July 2017) and Matters Arising
Apologies had been received from Councillor Wendy Makepeace-Browne, David Lispcombe and Karen Evans (Safer North Hampshire), Jessica Berry (NH CCG), Marie Baxter (Everyone Active), Nikki Seargent (Frimley Health), Sam Whiting (Hampshire Library Service)
The Minutes were agreed and actions considered as follows:
  • LG would follow up actions attributed to Nick Parkin
  • Gillian Trippner’s carers report had been circulated. SL queried the extent to which our local GP practices had been approached by the Princess Royal Trust. LG to find out which three (out of 19) haven’t had a visit.
  • Progress on EMIS system (HVA not present)
  • SL asked that the proposed Transportation Report be brought up with the CCG, as this had not been forthcoming. He suggested there was an urgent need for a review of patient transport under the health and wellbeing banner, especially in view of the budget reduction proposals from HCC which would have a knock on effect on patients needing transport. LG to consider.
  • Barbara Allen from the Wellbeing Service at Andover Mind reported that the anxiety support group in Hart would continue under the new contract.
  • Liz would look at the best place for the Autism Friendly Fleet project within the Hart Health & Wellbeing Plan

3 / Wellbeing Service (Centres) in North Hampshire
Jody Phelvin and Barbara Allenfrom Andover Mind, the new provider of the Wellbeing Service/Centres in North Hampshire,gave a presentation (Circulated).
The service will build on the current offer with introduction of wider range of workshops including sleep, relaxation, confidence building, and will take on the running of carers groups. The service will also adopt a hub and spoke model utilising local community venues to reach more people.
Questions and discussion included:
  • Transport –difficulty for Hart residents to get to the existing centres and the importance of outreach
  • Potential venues - community venues out of towns in more rural areas. The service has resource and capacity to set up spokes within a short timescale. Space may be available at Yateley Citizens Advice. Sally to liaise with Barbara.
  • Holistic approach to include families and carers
  • Citizens Advice seeing an increase in mental health clients
  • Supporting people with personality disorders.
  • Housing and tenancy sustainment
  • Trailblazer to connect with Andover Mind regarding defining a clear pathway

4 / Beating physical inactivity in Hart
Julie Amies, Chief Executive, Energise Me (previously Sport Hampshire & IOW), gave a presentation about tackling physical inactivity in Hart.
New data analysis has been carried out to inform development of a Hampshire Beating Inactivity strategy which will put forward a whole system approach.
Questions and discussions included:
  • The new data analysis highlights populations in Hartwhere there is a need to tackle inactivity levels and rising inactivity trends including women (particularly ages 35-44), disabled people and those with long term conditions, NS SEC 1-4 (inactivity level rising), big inequality between socioeconomic groups
  • How to bring about change
  • Community led involvement helps change
  • Funding tends to follow social deprivation which means a lack of funding opportunities in Hart
  • Big inequality gaps in the Hart area
  • Motivation of individuals and communities
  • Creating opportunities – not one solution fits all
  • Active 10 - NHS developed App – registering your activity (active 10 minutes of moderate exercise)
  • Are we working in areas of the districtwhere the leisure centres are less accessible e.g. Western parishes? Liz explained that HDC, HVA and Energise Me are working in partnership to engage local sports clubs and physical activity providers on the subject of tackling inactivity and that Everyone Active will be outreaching more into communities and then bringing people back into the Leisure Centre

5 / Fleet Healthcare Infrastructure
Adrian Wheeler, Estates Advisor, North East Hampshire & Farnham CCG, gave an update on the NEHants & Farnham health estates strategy. Main points included:
  • In 2015 CCGs and providers were tasked with looking at the estates strategy to reflect new models of care for the future. Ambitious, national timescale by end of 2015 which the CCG met.
  • The CCG consists of five localities Farnham (Waverley), Farnborough and Aldershot (Rushmoor), Yateley and Fleet (Hart). New models of care include more locally-based integrated care.
  • The health infrastructure (community hospitals, primary care etc) within those localities was looked at, with the focus around what the buildings did, using 5 criteria for assessment including availability, accessibility, suitability, sufficiency, affordability.
  • The CCG has assessed the estate in those terms and is using this to come up with a plan to develop and improve the estate
  • The CCG has worked closely with local authorities including HDC around the Local Plan development
  • The estates strategy says there is a need toexpand the health estate across the CCG area to meet expected population growth.
  • Integrated care services are being developed in each of the 5 localities. In Yateley a dental suite at Oakley Health has been converted into an urgent/integrated care hub.
  • A feasibility/options appraisal of sites has been carried out in Fleet and a number of options wereruled out includingremodelling the old Police building and building a new facility on the disused Park & Ride site at Elvetham Heath. The strategy in Fleet will be to build capacity in the existing 4 practices and the CCG is working with each practice on applying for minor improvement grants. The CCG recognises that Fleet Community Hospital has constraints in terms of transport and parking and needs updating. The CCG will work with partners to remodel the community hospital over time and a Sustainability & Transformation Partnership bid has been made to support investment.
  • Looking at local plans, when new housing developed S106 commitment to provide money by developers to contribute to future healthcare – Hartland Park etccould deliver over £1m of S106 funding

SC declared an interest at this stage as he is a GP at Branksomewood Healthcare Centre.
Discussion was around:
  • Synergyacross CCGs now that there is apartnership between 4 CCGs in Hampshire – AW reported that there is increasing joint working particularly with North Hants CCG around GP catchments and population growth
  • SF requested that there be wider engagement with Councillors, not just Planning Officers,as they are aware of the pressures within their wards etc. AW and SF to discuss outside the meeting.
  • Focus on improving the infrastructure we already have
  • JA asked whether there is potential for multi-purpose facilities linking leisure, primary care etc. AW reported that this has been looked at and there is some potential at Fleet Community Hospital within constraints.

LG agreed to bring forward the physical activity issues. Members agreed the data was important and we now need to look at how we make it happen. JA suggested that next meeting look at some of the suggested tactics for tackling physical inactivity which will form part of the strategy.
