SUBJECT:Faculty Annual Evaluation – Calendar Year 2010

Each institution in the SUS is required to conduct an annual evaluation of the performance of each faculty member. Faculty members are to be evaluated based on their assigned duties and the criteria established in the departmental merit pay plan. See the COEDU Annual Review Policy Statement (COEDU main page, Click on Faculty/Staff, Click on Annual Review, Click on policy statement – on right). This year’s annual review will cover the performance of faculty from January 2010 through December 2010.

Please complete your annual review using the FAIR system, which can be accessed from the USF NetID Single-SignOn – Login site or

the COEDU website (Click on Faculty/Staff; Click on FAIR system). It is necessary that your AFD/FAR percentage assignments for each semester in 2010 be completed, with at least some percentage in each category where you wish to report products and activities (e.g., teaching, instruction-related, advising, thesis/dissertation advising, research, service, university service, discipline-related service). This will enable you to enter data in the Annual Report Wizard Modules. You may enter research publications and presentations directly. It is also highly desirable for you to update your vita on FAIR with publications and presentations, as data are pulled centrally by the USF administration for use in its decision-making and resource allocations

Once you have completed entering your Annual Review, print a hard copy and submit it along with any supporting documentation consistent with your departmental policies and procedures – typically a current vita, publications (some departments also want presentations and papers submitted for review, along with documentation of receipt/acceptance of those papers by an editor). Deadline for submission of the electronic Annual Review and the hard copy along with supporting documentation to your Department Chairs is February 18, 2011.

Re-training on submission of the electronic Annual Review will be made available to faculty. Please contact Carol Didomenico who will meet with faculty in departments.

Timeline for Annual Review

By February 18, 2011Faculty members should complete and submit their annual reviews electronically via the FAIR system and Annual Report Wizard. By the same date a hard copy of the electronic annual review and supporting documentation (consistent with departmental policies and procedures) should be submitted to the Department Chair for review by the Departmental Annual Review Committee.

February 21, 2011 – March 25, 2011Departmental Annual Review Committee members reviewand discuss all annual reviews and supporting documentation, and write Evaluative Comments for each faculty member. For tenure-earning faculty, a section on Progress Toward Tenure must be written.

By March 28, 2011Departmental Annual Review Committee completes its reviews. The Chair of the Departmental Annual Review Committee pastes the evaluative comments into the appropriate section of Module 9 – Evaluations -- of the electronic Annual Review and completes the committee section of the College Annual Review Summary Report, including the electronic signature. The Committee Chair notifies the Department Chair that reviews have been completed, evaluative narratives posted, and electronic signatures completed.

By April 18, 2011Department Chair completes evaluative comments for each faculty member including Progress Toward Tenure (for tenure-earning faculty), pastes evaluative comments into the appropriate section of the electronic Annual Review (Module 9 – Evaluations), and completes the Department Chair section of the College Annual Review Summary Report, including the electronic signature. The Chair is to notify each faculty member that the Departmental Peer Reviews and Chair Reviews of the Annual Reports are completed and posted on the FAIR system. Faculty members arelogon to the FAIR system, read the review narratives and ratings, then sign electronically the Summary page. Note that faculty signature is merely an indication that the evaluative comments were received and read. The faculty member may indicate that he/she plans to submit a statement, which, when submitted, will be attached to the evaluative comments. Each faculty member is to notify via email the Chair that reviews have been read, that they electronically signed the summary page, and whether or not a reply has been written or is to be written.

By April 29, 2011Department Chair notifies the Associate Dean for Academic Affairs and certifies that for each faculty member in his/her department all documents (Annual Reviews, Evaluative Comments including Progress Toward Tenure), and electronic signatures of Chair, Peer Review Committee chair, and individual faculty member have also been submitted electronically.

An important step, following the above process, is for the Department Chair to meet with each faculty member to discuss continuing progress and performance (particularly tenure-earning faculty and faculty working toward promotion to full professor). Tenure-earning faculty and faculty working toward promotion to full professor are encouraged to meet with their mentors to discuss the feedback as well.

Timeline for Faculty Annual Reports (Calendar Year 2009)

By Friday, Feb 18, 2011 / Faculty members should complete electronically and submit hard copies of their on-line annual reports on the FAIR system, as well as the following:
  • Annual Review (Modules 1-8)
  • The on-line University Annual Faculty Review Summary form
  • Documentation file (as required by departmental guidelines)

By Monday, Mar 28, 2011 / Departmental Annual Review Committee completes Module 9 (called “Evaluation”) on-line, as well as the University Annual Review Summary Report form, for each faculty member. Committee Chair informs Department Chairperson that evaluations have been completed, evaluative reviews posted, and electronic signatures provided.
By Monday, Apr 18, 2011 / Department Chair completes Module 9 (“Evaluation”) for each faculty member:
  • Completes the on-line Annual Review Summary Report
  • Secures faculty electronic signatures on the Annual Review Summary reports.

By Friday, April 29, 2011 / Department Chair certifies to the Associate Dean for Academic Affairs that the Annual Report for each faculty member in his/her Department is completed. Specifically, the following tasks have been completed:
  • Annual Review of Faculty Activities Report
  • Evaluative Comments (Department & Chair)
  • Progress Toward Tenure (for tenure-earning faculty)
  • Faculty Self-Review Narrative or statement in response to reviews, if any
  • University Annual Faculty Review summary forms, with all electronic signatures
Faculty documentation files should be returned to faculty members.

An important step, following the above process, is for the Department Chair to meet with each faculty member to discuss continuing progress and performance (particularly tenure-earning faculty and those working toward promotion to full professor). Tenure-earning faculty members and those working toward promotion to full professor are also encouraged to meet with their mentors to discuss feedback as well.
