1This is a Career Break Agreement between Hampshire Fire and Rescue Service and

3The duration of the Career Break will be Year. Based upon this you will be due to return to work on. You must confirm in writing your intention to return to work no later than 3 months from the date you are due back to work. In your case this is.

4Your terms and conditions of employment remain in accordance with the particulars contained in your Contract of Employment dated 1st April 2006. All benefits, emoluments and allowances are however suspended by this agreement between the relevant dates given in sections 2 and 3 above including:

SALARY

Your current rate of pay is £per year. Upon return to work at the end of the Career Break you will recieve the salary applicable to the post to which you return. You will not receive a salary during your career break.

SICKNESS ALLOWANCE

You are not entitled to sickness allowance during the term of this Career Break.

LEAVE

Your leave entitlement is suspended during the term of this Career Break.

5NOTICE

The minimum period of notice of termination of employment which you are required to give remains in accordance with your contract of employment.

6RETURN TO WORK

You are entitled to return to work at the end of the Career Break in your current role but not necessarily in your current grade. Additionally you may not be able to return to your original post. Requests to return to work before the end of the agreed term may be considered subject to the availability of a suitable post.

7PENSION

Your position with regard to membership of the Firefighters Pension Scheme is a matter for you to take advice upon.

8GRIEVANCE PROCEDURE

Your rights to express a grievance remain in accordance with the procedures set out in SO/1/2/1 ‘Grievance Procedure’.

9DISCIPLINARY PROCEDURE

The disciplinary procedure remains applicable to you during the term of your career break.

10OTHER CONDITIONS

By entering into this Career Break Agreement you undertake not to take up any paid employment for the term of the Career Break unless specific written permission has been granted.

11All other terms and conditions of employment remain unchanged

I confirm that I have read and understood Service Order SO/1/15, Appx H, ‘Career Break Scheme’ and the implications regarding the career break I am taking.

A copy of this agreement must be forwarded to the HR Workforce Support Team at least one month prior to the start of the agreed Career Break period. The Workforce Support Team must also be advised immediately of any change to the planned return to work date.
