Mapping of Western Australian HACC services to the Commonwealth Home Support Programme

Background

The structure of the Commonwealth Home Support Programme (CHSP) is different to the structure of the Western Australian (WA) Home and Community Care (HACC) Program. The CHSP is made up of four Sub-Programmes:

  • Community and Home SupportSub-Programme
  • Care Relationships and Carer SupportSub-Programme
  • Assistance with Care and HousingSub-Programme
  • Service System DevelopmentSub-Programme

Each Sub-Programme has its own objective, eligibility criteria and service types. Under the CHSP funding agreement, service providers receive funding to deliver specified outputs against one or a combination of service types under each Sub-Programme.

Aged Care Planning Regions

Under the CHSP funding agreement, outputs and funding areallocated to Aged Care Planning Regions. This is a change from the WA HACC Program, which allocates units and funding to HACC Planning Regions. Services delivered in seven of the eleven WA HACC Planning Regions map directly to the corresponding Commonwealth Aged Care Planning Regions. These are the Goldfields, Kimberley, Midwest, Pilbara, Wheatbelt, Great Southern and South West regions.

There are minor differences in the boundaries of the East Metropolitan, North Metropolitan, South East Metropolitan and South West Metropolitan HACC Planning Regions and their corresponding Aged Care Planning Regions.

To ensure that CHSP funding agreements accurately reflect where services are provided, organisations are required to confirm the split of funding across regions is accurate in Workbook B. Where the transition from HACC Planning Regions to Aged Care Planning Regions changes the proportion of services delivered across regions, organisations are required to advise the split of funding for services across the Aged Care Planning Regions.

Mapping of WA HACC service types to the CHSP

The transition of WA HACC services for older people to the CHSP involves a process to map WA HACC service types to the structure of the CHSP.Information about current WA HACC service types has been analysed and compared to the CHSP Sub-Programme and service type descriptions to identify a best fit against the CHSP structure. The majority of WA HACC services map directly to the CHSP service structure.

The following table outlines the mapping of WA HACC service types to the CHSP.

Assessment / Assessment services will transition to My Aged Care – Regional Assessment Service from 1 July 2018.
Client Care Coordination / Funding for Client Care Coordination services will transition toMy Aged Care – Regional Assessment Service from 1July2018.
Counselling/Support Information and Advocacy (Care Recipient) / Advocacy services within scope of the National Aged Care Advocacy Program (NACAP) will transition to NACAP from 1July 2019.
Counselling/Support Information and Advocacy (Carer) / Carer support services will transition to the Integrated Plan for Carer Support Services, administered by the Department of Social Services from 1 July 2019.
Infrastructure (Service Group 8) / Purchase of land and major construction / capital works are outside the scope of the CHSP.

Note: A separate process will be undertaken to confirm the delivery of assessment arrangements in WA.
