DESCRIPTION OF BENEFIT

Preferred Provider Organization with 3 tiers of benefits providing both in-network and out-of network benefits.

Salary Income Protection for a long term illness that is based on years of service:

2-3 yrs. – 5 days 10-14 yrs. – 40 days

4-6 yrs. – 15 days 15-24 yrs. – 50 days

7-9 yrs. – 25 days 25-29 yrs. – 75 days

30+ years – 90 days

1 times annual salary up to $450,000.00.

Voluntary Term Life Insurance Program.

You can purchase amounts from 1 to 5 times your annual base salary.

DESCRIPTION OF BENEFIT

Flexible college savings program that helps make saving for a child’s higher education easier.

Voluntary payroll contributions are taken before Federal Income and Social Security taxes. Dependent Care Flexible Spending Account: Contributions can range from $500 to $5,000 annually. Health Care Flexible Spending Account:: Contributions can range from $300 to $2,000 annually.

25 days upon hire; 27 days after 1 year;
