eStatement Disclosure and Agreement

This agreement is between you and Green Belt Bank & Trust and provides your request and consent to receive statements and notices for your demand deposit account(s) by electronic delivery. These electronic statements and notices are called “eStatements”. By enrolling for eStatements, you are electing to receive your statement by email. Once enrolled, you will receive your very next statement electronically. Your normal statement cut-off date will not change when you elect to receive eStatements. Although you are opting out from receiving your statement(s)/notice(s) by U.S. mail, you may request a paper statement, at any time, fees may apply. You may return to receiving paper statements by contacting Green Belt Bank & Trust at 800-648-2544 or stoppingin at any of our locations. Location and additional telephone numbers are available on our website.

There are currently no service charge fees for the use of eStatements. However, you agree that Green Belt Bank & Trust has the right to institute or change the fees for eStatements upon ten days prior written notice to you. In addition Green Belt Bank & Trust has the right to amend these terms and conditions from time to time providing you with written notice of the amended terms and conditions. You agree to waive and release any claims against Green Belt Bank & Trust arising out of or that may in any way be related to the use of eStatements, except for those claims resulting solely from the negligent acts or omissions of the Bank. You agree that you are solely responsible for any loss due to use of eStatements by you, any authorized user, or any unauthorized user or recipient who gains access to eStatements through your computer or information obtained directly or indirectly from you. You must be an Internet Banking customer to enroll for eStatements and have at least one Green Belt Bank & Trust demand deposit account. Adobe Acrobat Reader version 6.0 or higher is required to open and view your statement(s)/notice(s).* To download the latest version of Adobe Acrobat Reader, http://www.adobe.com/products/acrobat/readstep2.html . We also recommend that you use the latest browser version available that supports 128 bit SSL Encryption. *Acrobat and Reader are registered trademarks of Adobe Systems Incorporated in the United States and in other countries. You will need email access with at least 1MB of free space to receive your statement(s)/notice(s). Statement(s)/notice(s) will be sent to the email address that’s on file with Internet Banking. We will notify you be e-mail each time your statement is available through Internet Banking. It is your responsibility to keep your e-mail address updated on Internet Banking. You have the option to have your statement(s)/notice(s) emailed to three additional recipients. It is your responsibility to maintain the ID, Password, and e-mail addresses for these additional recipients. With eStatements, you receive delivery of your statement(s)/notice(s). The delivery method used is a ‘Push’ method meaning your statement(s)/notice(s) are emailed to you as a secure .PDF (Portable Document Format) attachment. Statement(s)/notice(s) are sent as a “shell” .PDF without customer data. Once the user logs into eStatements, the ID and Password are authenticated and the data fills the PDF document. The data included in the statement(s)/notice(s) is streamed via a secure 128-bit SSL encryption. Your internet banking ID and Password are your keys to eStatements. It takes both identification keys to gain access. No one representing Green Belt Bank & Trust will ask for your Password. You should never give your password to anyone who asks for it in an email or by phone or anyone else who you do not want to have access to your account. Statement(s)/notice(s) can be printed or saved to a file on your computer for easy storage. The past two months statement(s)/notice(s) will be available on Green Belt Bank & Trust internet banking for your convenience. You may receive images of your checks with your electronic statement depending on the account type that you have.

In Case of Errors or Questions About your Account Statement

You must promptly review your electronic disclosures, eStatements and any accompanying items. Notify us by writing to us at 616 Washington Avenue, Iowa Falls, IA 50126; calling us at 641-648-2544 or 1-800-648-2544 or e-mail us a secure email if you think your statement is wrong of if you need more information about a transfer on the statement. We must hear from you no later than 60 days after we made available the FIRST statement on which the error or problem appeared. Tell us your name and account number, describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe there is an error or why you need more information. Tell us the dollar amount of the suspected error.

We will investigate your complaint and will correct any error promptly. If we take more than 10 business days we will recredit your account for the amount you think is in error; so that you will have use of the money during the time it takes us to complete our investigation.

In Case of Error or Questions about your Electronic Transfers

If you need more information about an electronic transfer appearing on this statement, or if you think your statement or receipt is wrong, please telephone or write us a soon as possible at the phone number or address designated above. We must hear from you no later than 60 days after we sent you the FIRST statement on which the error or problem appeared. Tell us your name and account number (if any), describe the error or transfer you are unsure about, and explain as clearly as you can why you believe there is an error or why you need more information. Tell us the dollar amount of the suspected error. We will investigate your complaint and will correct any error promptly. If we take more than 10 business days to do this, we will recredit your account for the amount you think is in error, so that you will have use of the money during the time it takes us to complete our investigation. If you would like to confirm that an automatic deposit to your account has been made as scheduled, you may call us during normal business hours at 641-648-2544.

Explanation of Balance on Which Finance Charge is Computed

We calculate the Finance Charge on your account by multiplying the daily balance of your account, including current transactions, by the daily periodic rate each day during the billing cycle. This gives us a daily finance charge. Then we add together each daily finance charge to derive a total Finance Charge for the billing cycle. To get the daily balance on which each daily finance charge is computed, we take the beginning balance of your account each day, add any new advances and subtract any payments or credits and unpaid finance charges. The “average daily balance” shown on the statement is for purposes of illustration only. To validate the amount of your finance charge, multiply the number of days in the billing cycle by the average daily balance shown, then multiply the product by the daily periodic rate. *Note: If the statement closing date falls on a Friday or on any business day immediately prior to a non-business day, the number of days in the billing cycle will include the subsequent number of non-business days until the next business day, and the finance charge will continue to accrue. However, the number of days in the next billing cycle will not include any days included in the prior cycle.

By accepting this Agreement, you are indicating that you have read, understand and agree to be bound by the agreement and that you, and any authorized signers or users authorize us, at our discretion, to provide electronic delivery of statements and other bank related documents. You understand this agreement is effective at the time of enrollment.
