Graduation Checklist 2015

We are providing you a checklist to help you through this busy time. More information will follow regarding the ceremony (place, time, seating arrangements, etc.). Please watch your email and mailbox closely from now till graduation.

_____1.Complete application for graduation (exactly how you want your name printed on diploma). Located on AccessPlus, Vetzone and/or Graduation website by October 10.

_____2.Complete themandatory Financial Aid Exit Interview on-line. More information with link and details will be sent to you via email. Clay Gurganus will also speak to you during the week of May 5. Watch for email.

_____3.Graduation announcement order information. Samples are on display in the OASA office. Go to call 1-800-433-0296. Representative will be here in February.

_____4.Vote for 2 marshals to attend graduation with your classtoescort the class into and from the auditorium. Nomination forms due back to OASA office by March5.Link will be on Vetzone.

_____5.Complete Zoetis Distinguished Teacher Award Nomination on Blackboard. Watch for email with instructions. Voting will be open March 24-March 31.

_____6.Was your faculty contactor another contact helpful to you over the years? Nominate him/her for the William O. Reece Academic Advising Award. Nomination form will be done onBlackboard. Watch for email. Voting will be open March 24-March 31.

_____7.Order Cap & Gown with Willsie on-line at:

Deadline: must be processed by April 3 to avoid a late fee.Costapprox.. $61. Caps and gowns will arrive for pickup Friday, May 8th in the Alumni Roomfollowing rehearsal. If you are unable to pick up your gown at the vet school, it will be taken to Stephens Auditorium for you.

_____8.Loans Receivable exit interviews are done online at or go to the main ISU home page, click on loans receivable, then exit interview. This is for anyone withHealth Professions loan, Perkins, Disadvantaged Student loan, or Univ. Long Term.

_____9.Complete “Placement Information/Salary survey report” form only by April 20. Your name is kept in strict confidence. A report is generated from this that shows the average salary for your class as a whole, and then broken down by practice type, # employed, states of employment and other valuable information. Diplomas may be held if not completed.

_____10.If you are graduating with distinction (3.50 or better), your honor chord will be placed in with your cap and gown.

_____11.Take Computer to IT to be reconfigured. Must be done by May 8.

_____12.You will be asked to verify your name as it appears in the graduation book and on your diploma with the phonetic spelling for the person reading your name at the ceremony to save any embarrassment of having your name pronounced incorrectly. Watch for email.

_____13.Clean out locker and mailbox.Return books to library and pay all fines.

_____14.Apply for DEA license if necessary (this can’t actually be done till after graduation). Go to

_____15.Turn in all university keys and key cards. There will be a $20 charge for any lost or non-returned keys.

_____16.Graduation Rehearsal: Friday, May 8, 2015 from 9:00-10:00am, room 1226.

Financial Aid Q&A will be held in conjunction with rehearsal. More information will be sent via email.

_____17.Attend graduation. The College of Veterinary Medicine diploma awarding ceremony will be held in C.Y. Stephens Auditorium at 12 noon on Saturday, May 9and will last one hour. The auditorium doors will be open about 11:15 a.m. There is no limit on the number of guests that may attend. Highlights of the ceremony will be hooding on stage, a charge to the class, the president of the Iowa Veterinary Medical Association will administer the veterinarian’s oath, and diplomas will be awarded by the Dean and the University President.

A photographer will be present from Flash Photography that will take your picture as you receive your diploma. You will be emailed the link to order pictures from. The ceremony is not video taped, but your family may do so if desired. More detailed ceremony instructions will be given at a later date.

Please notify me 2 weeks or more prior to ceremony if you have handicapped guests (that cannot go up or down stairs) or hearing impaired that requires the need for sign language during the ceremony. (complete form online)

_____18.Pay all outstanding traffic tickets, library or health fees, and a graduation fee charged to your account approx. 1 month prior to graduation inRoom 1220Beardshear.

_____19.Upon employment, have your proof of accreditation paper signed by the state veterinarian in the state in which you are practicing.
