PARENT / ATHLETE CONTRACT

As a parent of a Gateway Regional High School Athlete, I am proud of my son/daughter and I want to work with Gateway to help my athlete be all that he/she can be. I understand that society places a great deal of pressure on young athletes, and negative aspects of our society can seek to influence them. As a parent, I will support Gateway’s efforts to provide a positive winning environment for my athlete by:

  1. Not permitting or condoning the use of alcohol by student-athletes on my property, and to encourage other parents to do the same.
  1. Not permitting or condoning student-athletes participation in any hazing or initiation activities with teammates or other students.
  1. Encouraging athletes to adhere to all the training rules set by the coach.
  1. Following the appropriate chain of command if there is a problem on the team, seeking the resolution with the coach and Athletic Director first.
  1. Exhibiting good sportsmanship at all athletic events.
  1. Supporting the good of the team and not allowing selfish interests to hurt the team concept.

Gateway is proud of all of its student athletes and I am proud to be a part of their athletic program.

PARENT’S CONSENT FOR

I hereby give my consent for my son/daughter to participate in the High School Athletic Program sponsored by

Gateway Regional High School.

Realizing that such activity involves the potential for injury which is inherent in all sports, I acknowledge that even

with the best coaching, use of the most protective equipment, and strict observance of rules, injuries are still a

possibility. On rare occasions these injuries can be so severe as to result in total disability, paralysis, or even death. I

acknowledge that I have read and understand this warning. In addition, I give permission for my child to be treated

in the event of such an injury by a certified athletic trainer, school nurse, EMT’s, paramedics and/or hospital. I also

agreeto not hold GRHS responsible for any injuries that may occur during sports participation.

The school medical inspector has my permission to examine my son/daughter in the presence of a school nurse if

necessary.

Gateway Regional High School District has a comprehensive insurance plan for student athletes. It is provided as

“secondary coverage.” For example, the parents’ insurance carrier is responsible to the limit of their coverage.

Thereafter, the school insurance will pick up the excess charges. If the parents do not have insurance, the school

insurance becomes the primary coverage.

STUDENT’S RESPONSIBILITY

  1. As a high school student athlete, I understand that academic eligibility for participation in extra-curricular activities is determined twice during the school year. For the first semester, September 1-January 30, eligibility is established on September 1. Eligibility for the second semester, January 30-June 30, is established January 30, as per NJSIAA rules.
  • Newly placed ninth grade students are automatically eligible during their first semester
  • Returning students in grades 10 and 11 must have earned 30 credits in the immediate preceding academic year. Summer school grades or those from an approved tutor may be used in determining eligibility. Eligibility for the second semester (January 30-June 30) for all grades, 9-11, is dependent upon a student passing 15 credits during the immediately previous first semester and 13.75 credits for students in 12th grade.
  • Students carrying less than 27.5 credits (early release) in any semester must pass all courses on their schedule.
  • Once a student is determined to be academically ineligible for any semester, that student may re-establish eligibility in the next semester by meeting the requirements detailed above.
  • For high school athletes, NJSIAA rules allow athletes who are participating in a particular sports season to finish their season if they should become ineligible mid-year. The student athlete would then be ineligible for the next sports season.

2I recognize my responsibilities if I try out for a school athletic team. I will make it a point to so govern myself that my connection with the sport will bring honor to it and the school, and expect to be asked to withdraw from the team in case I do not.

  1. I also acknowledge that the equipment loaned to me by the school must be returned at the end of the sports season. If the equipment is lost or misplaced due to my negligence, I agree to pay for that equipment which is not returned.
