Ohio Graduation Test Preparation

Summer 2013

Program Description: Students enrolled will be tutored within a specific subject area and will be eligible to participate in a special test session of the Ohio 10th Grade Graduation Test. Students will be permitted to take tests in the areas in which they have completed preparation courses (Reading, Math, Writing, Science, Social Studies). There is no credit given for the course. Ten (10) class hours are required to sit for the test(s). If a student has less then 10 hours of remediation in any of the subject areas they are not eligible to take that section of the OGT.

Eligible Students: All current 11th and 12 grade students who have failed one or more sections of the OGT.

OGT Remediation Classes will be held 4 days for 1 week, beginning on Tuesday, June 11th and run through Friday, June 14th . Remediation classes will run according to the schedule below.
The week of June 17 students will have an opportunity to take the test. The order of the tests will follow the schedule below. The fee for OGT Remediation is $50.00 for each course.

REGISTRATION INFORMATION

SUMMER SESSION OGT REMEDIATION COURSES:
MONDAY, JUNE 10 – FRIDAY, JUNE 21, 2012

1. Registration will begin Monday, May 20, 2013 and run through Friday, June 7, 2013.
Completed forms with accompanying payment must be received NO LATER THAN 2:30 P.M. on Friday, June 7, 2013. These must be brought in person to the Garfield Heights High School Guidance Office.

2. All class assignments will be made on a first-come first-served basis.

3. Registration must be paid in full with cash, credit card, check or money order made payable to

Garfield Heights Board of Education.
Returned non-sufficient fund checks will result in an additional assessed fee of $35.00.

4. There will be absolutely no registration accepted after Friday, June 7, 2013 for OGT Remediation

Classes.

REFUNDS/CANCELLATIONS

1. GHHS reserves the right to cancel any class. Refunds are issued according to the following:

a. Full refund if the course is cancelled by the school district.

b. A processing fee of $20.00 will be charged for each class dropped.

c. There will be no refund after: June10, 2013

2. Allow 4 weeks for refund checks to be processed by the Treasurer’s Office and mailed to the home address

indicated on the Registration Form.

3. Since the State of Ohio mandates a student receive 10 hours of remediation in order to be eligible to take the
summer OGT tests, it is mandatory that the student be present and on-time for all 4 days. There is no refund
due to a student absence.

STUDENT CODE OF CONDUCT

GHHS Summer OGT Remediation/Credit Recovery has adopted a code of conduct. Violations of school rules and regulations, during the time a student is under the auspices of school personnel and/or authorized volunteers, may result in disciplinary action, including, but not limited to, the removal from OGT remediation without tuition refund and the student will not be able to sit for the test. The kinds of misconduct likely to result in this consequence are included in the attached Student Code of Conduct and must be signed and returned with the registration.

GARFIELD HEIGHTS HIGH SCHOOL IS A CLOSED CAMPUS. Students may not leave the building without permission, for any reason. Any student leaving the building without permission may be subject to removal from the OGT remediation program without refund or the ability to test.

ATTENDANCE

Because the State standards govern the number of hours a student must be in class to sit for the test, daily attendance is mandatory.

ILLNESS/INJURY

If any student becomes ill, the school will attempt to notify parents/guardians to make any necessary arrangements. Serious illness or injuries will be reported to 911.

STUDENT CODE OF CONDUCT

Any school that does not have appropriate student behavior will not operate effectively. Good student behavior is everyone’s responsibility. This means that students, teachers, administrators and parents have an important part in developing proper attitudes and behaviors. It is extremely important for students to take responsibility for their actions.

A violation in any of the following areas may result in a suspension from the OGT remediation program.
NOTE: Suspensions will result in dismissal, per our attendance policy, with no refund of fees and inability to take the test.

1. Using or possessing tobacco products.

2. Using or possessing alcohol or drugs or being under their influence.

3. Damage of any property.

4. Using inappropriate and/or abusive language (racial, sexually abusive language, making offensively coarse
utterances, gestures or communicating unwarranted and abusive language, harassing, instigating, or provoking

others).

5. Insubordination.

6. Assault.

7. Making, selling or possessing counterfeit drugs and/or related paraphernalia.

8. Fighting.

9. Setting false fire alarm or bomb threat.

10. Possessing, handling, or transmitting dangerous weapons or look-alike weapons.

11. Refusing to identify oneself to authorized school personnel.

12. Possession of electronic devices.

13. Theft of private or public property.

14. Producing or assisting in producing a flame.

15. Returning to car/parking lot without permission.

16. Creating an unsafe situation for oneself, others or the facility.

17. Failing to follow arrival/dismissal procedures.

18. Sleeping in class.

19. Clothing deemed inappropriate by administration including those with negative connotation.

20. Excessive and/or inappropriate public display of affection.

21. Forgery of school related documents.

22. Cheating on exams and/or school work.
