Prom 2016 Agreement & Guest Application

I do hereby agree as a condition of attending the GMC prom and/or for bringing a guest:

1.  I will follow rules and regulations set by GMC.

2.  I understand that poor conduct will not be accepted.

3.  I will not use any form of tobacco, drugs, or alcohol while on the premises.

4.  If evidence of any form of tobacco, drugs, or alcohol use before/during prom, I will be subject to disciplinary actions.

5.  I will not possess any weapons on prom grounds.

6.  I will be dressed appropriately:

-  Gentlemen in pants/coat/tie or tux

-  Ladies in dresses or formals (lower back, midriff and excessive cleavage should be covered. Skirts or splits cannot be above mid-thigh) Questions about dress should be addressed with Mrs. Bailey or Ms. Williams.

7.  I will not leave prom with plan of re-entry. Upon exiting building, I will promptly leave the premises.

8.  I understand guests must be between the ages of 14-20 (no middle school students).

9.  I agree that violation of any aspect of this agreement is sufficient cause to be subject to disciplinary action, including arrest.

10.  I understand that only one guest form can be submitted per student.

11.  Tickets will be sold in advance and must be shown to enter prom.

12.  I understand that I must show a picture ID to be admitted into the dance. My guest must be approved to purchase a ticket and must show picture ID to enter the dance.

13.  I understand I must sign and complete all of this form for approval by Mr. Armstrong.

***Must turn in for approval no later than March 25th to Mrs. Bailey in B3. Tickets will be given when form is turned in***
