1)Students who intend to enroll in SM4701 Graduation Thesis/Project for BScSCM FYP (Phase I)are required to complete this Declaration Form. Once approval is granted on this Project Proposal, the School will register the course on your behalf.
2)BScSCM treat “SM4701 Graduation Thesis/Project” and “SM4602 BScSCM Graduation Project” together as a one-year graduation project (FYP) separated into two phases. Each phase carries 3 credit units. Student must get a pass grade in SM4701 in order to successfully register into SM4602. The same pair of SCM and CS advisors will supervise student for a one whole year. Before the summer, students are required to select and consult with their desired FYP supervisors, and start performing their ideation, research & development, proposal writing and project planning through out the summer. Those outcomes will need to be injected into SM4701 (BScSCM FYP Phase I – Pre-Production) and SM4602 (BScSCM FYP Phase II – Production);
- BScSCM FYP Phase I – Pre-production: SM4701 is the pre-production phase for SM4602. This course is focus on Project Management; Research & Analysis; Insight & Concept Development; Feasibility Studies; Usability Studies; Experiment & Testing; Mock Up making & Prototyping; Pre-Visualization; and Assets Management & Preparation for SM4602.
- BScSCM FYP Phase II – Production: SM4602 is the production phase based on the outcomes from SM4701, focus on Project Management; Production; Assets Management; Debugging; Optimization & Fine-Tuning; Documentation and Presentation.
3)Students must obtain potential Advisors’consent and signature before submitting this form. The School is NOT responsible for arranging Advisors for any students. The project will be co-supervised by CS and SCM staff. There should be at least ONE Advisor from each of CS and SCM. Please refer to the Attachments for the lists of CS and SCM staff members and their areas of interests who may take up the role as Advisors for SM4701 Graduation Thesis/Project for BScSCM FYP (Phase I).
1)Please submit the completed Declaration Form on or before 2013-2014 Summer - Week 23, 23th – 27th Junein personor by post toSchool of Creative Media or by e-mail .
2)Advisors may contact students for modification of the project proposal, if required, before students could formally start the Final Year Project in Summer Term, 2013/14.
3)Students can only change the Advisors and FYP topic within the first week of Semester A, 2014/15with strongjustification and approval from course leader. You need to fill in this form again and inform the General Office of the changes.
Major changes after this period are NOT RECOMMENDED. If you insist, you are required to provide substantial justifications for the School’s re-consideration.

Notes to Students who are going to exchange in semester A/B
Pleases note that it is not a normal practice to take a course while you are on exchange.Hence, priorapproval from the School is required. If you intend to register for FYP while on exchange, you need to provide the supporting statement from your potential supervisors, and submit the justification with this form to Programme Leader (Dr. Fu, Hong Bo) and Associate Dean (Prof. Christian Wagner) for consideration.Among other things, the following information is required:
- What is the nature of the project (group or individual)?
- What is the role of the student if it is a group project?
- What contributions can the student perform in the semester on exchange?
- What monitoring mechanism will be in place to ensure student's participation in the project before and after the exchange?
- Whether the registration of FYP while on exchange is supported by potential supervisors?

If there is any BA/BAS student(s) in the group, please also fill in the name(s) and state “BA/BAS” after the name(s).(Important Note: Since the BA/BAS’s FYP implementation is different from BScSCM implementation, - BS/BAS student have no dedicated course for pre-production phase in semester A, so grouping with BA/BAS student should put more efforts in project planning and cooperation)
