In collaboration with the Dependency Court the following change in court report writing allows for the Health and Education Passport (HEP) to be attached to court reports in lieu of the narratives for medical, dental, education, developmental and mental health status currently being submitted for court’s review. In the interest of efficiency, adherence to state and federal regulation( 42 U.S.C. Section 675 (5)(D), and our existing departmental policies, a child’s HEP is to be reviewed, updated, and provided to the caregiver(s) at time of placement and transition from care. To ensure timely, easily accessible and proper documentation of our children’s health and education the Dependency Court henceforth recognizes and accepts the HEP as the primary archival system and means to update court on our children’s overall well-being. All prior court expectations for updates on a child’s health and well-being are still required for the court’s review and must be reflected in the attached HEP, per Procedural Guide 0080-505.20, Health and Education Passport. The HEP entries are archival and the information included must be limited to reports and summaries from agencies and experts in their particular fields; ie, physician’s reports, clinic reports, therapist’s reports, RegionalCenter annual reports, and IEP plans. Additional pertinent information from observers who are not expert in the field being reported should be included in the Current Family Circumstances portion of the reports to the court.
The Education Section has been rolling out Education Rights and technical training to social workers department-wide, as well as technical training and support for clerical staff to assist in entering education information. The court has already begun accepting attached Health and Education Passports from social workers who have attended the training. This new protocol for a comprehensive system of recording the progress of our children and the services they are receiving in their Health and Education Passports will inform the court, while enabling social workers, supervisors and administrators quick access to the information from one common source.
