Instructions

Section 618(b) of P.L. 101-476 authorizes the Secretary

to collect such information as is necessary for the implementation of this Act.

Description of Demographic Student Data Reported on All Lists:

The following data items are on each student list and must be reported for all students who meet the reporting requirements for a given data collection. Failure to include any of the data elements below will result in a data error for that student. Please make sure that all data fields or columns have the appropriate information.

The Demographic information below is produced from Infinite Campus Reports for both the Exiting Report and the Ad hoc queries designed to produce sections, 3, 4, and 6 of the Discipline Data. Sections 1 and 2 must be produced manually on the EOY File.

District Number: This is the three digit number assigned to each district by the Kentucky Department of Education.

School Number: This is the three digit number that when combined with the district number gives a unique school ID to each school in the state.

State Student ID: Also referred to as the SSID, this is the unique student identification number assigned to each enrolled student in Kentucky. District must use the State Student ID this year.

Student Name: There are separate fields for Last Name and First Name. Middle name if any should be included in the first name field if necessary to distinguish this student from another student similarly named.

Date of Birth: This field is the student’s date of birth (DOB). The date should be entered with the birth month first, followed by the day and year. There must be a ‘/’ or a ‘-‘ entered between both the month and day and again between day and year (mm/dd/yy). Month is entered as a number, for example, ‘01’ for January. Do not enter text. Failure to enter a ‘/’ or a ‘-‘ between the month and day and between the day and year is incorrect and the date will be rejected.

Gender: This field is a drop down box and the user must select from the list provided for every child included in the report. Gender options in order of appearance are:

F - Female

M - Male


Ethnicity: This field is a drop down box and the user must select from the list provided for every child included in the report. Ethnicity or Race options in order of appearance are:

WH - White, Non-Hispanic

This race identifies a person having origins in any of the original people of Europe, the Middle East, or North Africa.

BL - Black, Non-Hispanic

This race identifies a person having origins in any of the Black racial groups of Africa.

HI - Hispanic

This race identifies a person of Mexican, Puerto Rican, Cuban, Central or South American, or other Spanish culture of origin, regardless of race.

AS - Asian or Other Pacific Islander

This race identifies a person having origins in any of the original peoples of the Far East, Southeast Asia, the Indian subcontinent, or the Pacific Islands. This includes for example, Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Philippine Islands, Thailand, Vietnam, Hawaii, Guam, and Samoa.

AM - American Indian or Alaska Native

This race identifies a person having origins in any of the original peoples of North and South America (including Central America) and who maintains tribal affiliation or community attachment.

PI - Pacific Islander

This race identifies a person having origins in any of the Pacific Islands including Natives of Hawaii.

MU - Two or More Races (Multiple Races).

This race identifies a person who can be identified under two or more of the categories above.

LEP Status: This field is a drop down box and user must select from the list provided for every child in the report. LEP Status refers to whether the child meets the definition of a Limited English Proficient child under the Elementary and Secondary Education Act. LEP Status options in order of appearance are:

NLEP This student is not receiving or eligible for LEP Services

IFEP This student has been identified as ‘Initially Fully English Proficient’

LEP This student has been identified as ‘Limited English Proficient’

RFEP This student has been identified as ‘Redesignated Fully English Proficient


Disability: This field is a drop down box and the user must select from the list of disabilities provided for every child included in the report. Disability options in order of appearance are:

01 - MMD or Mild Mental Disability

02 - FMD or Functional Mental Disability

04 - HI or Hearing Impaired

05 - SL or Speech Language

06 - VI or Visually Impaired

07 - EBD or Emotional Behavior Disability

08 - OI or Orthopedically Impaired

09 - OHI or Other Health Impaired

10 - SLD or Specific Learning Disability

11 - DB or Deaf Blind

12 - MD or Multiple Disabilities

13 - AUT or Autistic

14 - TBI or Traumatic Brain Injury

Report of Children with Disabilities Subject to Removal to an IAES

Definitions of Terms used in the Disciplinary Removal data:

Dangerous Weapon: A weapon, device, instrument, material, or substance, animate or inanimate, that is used for, or is readily capable of causing death or serious bodily injury, except that such a term does NOT include a pocket knife with a blade of less than 2.5 inches in length (18 U.S.C. Section 930 (g)(2)). (Section 1)

Disciplinary Removal: Any instance in which a child is removed from his/her educational placement for disciplinary purposes, including in-school removal, out-of-school removal, expulsion, removal by school personnel to an interim alternative education setting for drug or weapon offenses or serious bodily injury, and removal by hearing officer for likely injury to self or others. (All Sections)

Drug Offenses: The use, possession, sale, or solicitation of drugs as identified in 21 U.S.C. Section 812(c). These offenses do NOT include use, possession, sale, or solicitation of alcohol or tobacco. (Section 1)

Expulsion: An action taken by the local educational agency removing a child from his/her regular school for disciplinary purposes for the remainder of the school year or longer in accordance with local educational agency policy. Include removals resulting from violations of the Gun Free Schools Act that are modified to less than 365 days. (Section 6)

In-School Removals: Instances in which a child is temporarily removed from his/her regular classroom(s) for disciplinary purposes but remains under the direct supervision of school or district personnel. Direct supervision means school personnel are physically in the same location as students under their supervision. (Section 4)

Interim Alternative Educational Setting (IAES): An appropriate setting determined by the child’s ARC (IEP team) in which the child is placed for no more than 45 school days. This setting enables the child to continue to receive educational services and participate in the general education curriculum (although in another setting) and to progress toward meeting the goals set out in the IEP. As appropriate, the setting includes a functional behavioral assessment and behavioral intervention services and modifications to address the behavior violation so that it does not recur. (Sections 1 and 2)

Out-of-School Removals: Instances in which a child is temporarily removed from his/her regular school for disciplinary purposes to another setting (e.g., home, behavior center). This includes both removals in which no IEP services are provided because the removal is 10 days or less as well as removals in which the child continues to receive services according to his/her IEP. (Section 3)

Removal by a Hearing Officer: Those instances in which an impartial hearing officer orders the removal of children with disabilities from their current educational placement to an appropriate alternative educational setting for not more than 45 school days based on the hearing officer’s determination that maintaining the child’s current placement is substantially likely to result in injury to the child or others. The ARC (IEP team) is responsible for determining the interim alternative educational setting. (Section 2)

Serious Bodily Injury: A bodily injury that involves a substantial risk of death; extreme physical pain; protracted and obvious disfigurement; or protracted loss or impairment of the function of a bodily member, organ or faculty (18 U.S.C. Section 1365(3(h)). (Sections 1 and 2)

Unilateral Removal: Instances in which school personnel (not the ARC/IEP team) order the removal of children with disabilities from their current educational placement to an appropriate interim alternative educational setting for not more than 45 school days. The ARC (IEP team) is responsible for determining the interim alternative educational setting. Unilateral removals do NOT include decisions by the IEP team to change a student’s placement. (Section 1)

Discipline Data – Section 1: Unilateral Removal by School Personnel to an IAES

Specific Instructions (to be completed manually)

1.  This list should include special education students who were removed from school by school personnel for drugs, weapons, or serious bodily injury and who were sent to an Interim Alternative Educational Setting (IAES) for 45 days or less. This includes special education students who were sent to an IAES for 10 days or less. Special education students who were removed from school by school personnel for drugs, weapons, or serious bodily injury for 10 days or less and were NOT sent to an IAES should be reported as having been suspended either in Section 3 (Out-of School) or Section 4 (In-School) depending on the type of that removal. Do NOT include these children in this section unless after this removal the student was also subject to a unilateral IAES removal.

2.  Report a student separately for each time the student was unilaterally removed to an IAES for Drugs, Weapons, or Injury in the columns under the heading of ‘Removals to an IAES Only’. If the incident that resulted in the removal to an IAES, involved Drugs, Weapons, and or Injury, select the reason that was most egregious for this incident.

3.  If a special education student was unilaterally removed to an IAES more than once for the same reason, then that student should be entered separately for each removal.

Discipline Data – Section 2: Removal by a Hearing Officer to an IAES

Specific Instructions (to be completed manually)

1.  This list should include special education students who were removed from school to an IAES by a Hearing Officer determination of ‘Likely Injury to Self or Others’.

2.  Students sent to an IAES more than once by a Hearing Officer determination of ‘Likely Injury to Self or Others’ should be reported separately in this section for each removal.

3.  To report a special education student on this list, provide the required demographic information and report the total number of days the student has been removed. This section is only for students who were removed to an IAES by a Hearing Officer determination of ‘Likely Injury to Self or Others’.

Remaining sections of the Discipline Data Requirement are to be produced from Infinite Campus. These CSV extracts created by the Ad hoc queries should be uploaded to the Secure File Transfer (DoSE Upload).

In the column labeled ‘Data on the Report’, select ‘Yes’ if there is data for the specified report or ‘No’, if there was no data.

In the column labeled, ‘Date Submitted’ report the date that file was uploaded.

The 2011-2012 EOY Data File for special education along with the various reports described above are due to the Division of Learning Services via the Secure File Transfer (DoSE Upload) web application no later than July 31, 2012.
