TheCollaborative Care Initiative:

Goal 1.Expand the Collaborative Care Initiative: A Partnership between Pediatrics and Psychiatry(referred to hereafter as “The Initiative”), which improves the delivery of child mental health care on a population level.

Objective 1-1. Expand the capacity of primary care providers to identify and manage children and adolescents with mental health problems.

  • Collaborate with the existing 100+ primary care providers from 18+ separate practices in North Florida
  • Develop and pilot an up-to-date workshop on the identification, assessment and management of depressionand suicide risk in youth.
  • Additions to the workshop will include a focus on special populations:
  • LGBTQI+ youth
  • gender nonconforming youth
  • The new workshop will also provide additional tools for the management of depression (i.e. web-based applications, therapeutic workbooks, etc.)
  • Create new standardized patient case scenarios that allows primary care providers to address the unique needs of gender nonconforming and sexual minority youth
  • Recruit and train a cast of standardized patients who can portray the roles of adolescents with depression, suicide risk, and/or who are gender nonconforming and sexual minorities
  • Provide ongoing support(including clinical site visits) to assist CCI PCPs with mental health care

Measurable Goals
Workshop on youth depression and suicide risk for special populations developed and piloted with primary care providers (#36)
Cast of standardized patients (#20) recruited and trained
New standardized patient case scenarios (#2) developed
Primary care providers (total # 100) who have participated in CCI training to care for children and adolescents with mental health problems

Goal 2. Increase the number of engaged CCI clinicians (i.e. PCPs and child and adolescent psychiatrists)in order to expand clinical services and access to care for youth with mental health problems.

Objective 2-1. Expand the access to child and adolescent psychiatry clinical consultation for PCPs through the Collaborative Care Initiative (CCI) Consultation Clinic.

  • Expand the capacity of the CCI Consultation Clinic at the University of Florida by 50%

Measurable Goals
Total patient slots (hours) available each month for CCI Consultations at UF (Total #15)
Total psychiatrists available to see CCI Consultations at UF (Total #5)
Total primary care providers invited to refer patients to the CCI Consultation Clinic (total #50)
Total primary care provider who refer at least one patient for consultation in the CCI Clinic (total #25)

Objective 2-2.Build the skills ofthe new pediatrician(s) and leader(s) of the Pediatric Wellness Center (tbd), to Integrate Mental Health Care in Pediatrics. Dr. Fallucco will provide consultation, leadership, and mentorship for this/these pediatrician(s) which will include regular clinical site visits.

Measurable Goal
Total meetings and site visits (#20) with Pediatric Wellness Center pediatrician (tbd) for consultation, mentorship

Objective 2-3. Train pediatric residents to develop skills in providing mental health for children and adolescents. This will effectively expand the capacity of the pediatric workforce of the future to deliver mental health to youth across the country.

Measurable Goal
Weeks of training provided to pediatric residents in Child Mental Health (goal #40 weeks)

Goal 3. Conduct assessment and quality improvement projects to determine how these novel Medical-Behavioral Health homes impact PCP practices in mental health care.

Objective 3-1. Using quality improvement models, examine how the CCI Consultation Clinic affects access to child mental health care for children with mild-moderate severity mental illness.

Objective 3-2. Using quality improvement, enhance the capacity of the Pediatric Wellness Center to care for youth with severe mental illness.

Objective 3-3. Expand our website on the CCI Model, to enable primary care providers to access educational materials, referral information. This could serve as a model for other national sites seeking to replicate and build upon our Center’s model.

Provide quarterly reporting of progress toward goals and objectives including data collected.
