1 / Proof of Citizenship/Qualified Alien / Photo ID for Head of Household ANDSocial Security cards for all members of your household.If you don’t have a Soc. Sec. card: bring a birth certificate, valid U.S. Passport, Certif. of Naturalization (N-550 or N-570), Certificate of Birth Abroad of US citizen (Form FS-240 or Form FS-545)
Qualified Alien documentation: Alien registration card (Form I-551); Unexpired Temporary I551 stamp in foreign passport; INS Form I-94; Employment Authorization Card (INS Form I-688B), or Employment Authorization Document (INS Form I-766)
2 / Utility Bills / All current utility bills with your service address and account numbers on them. You must submit a copy of your primary heating bill. If it is a delivered fuel, please submit a copy of the delivery slip or an invoice from the vendor.
3 / Income / Last 4 weeks gross income for everyone in the household 18 years and older. All income MUST BE documented, including: payroll, unemployment, child support, odd jobs, Veteran’s Benefits, pensions, AFDC, Social Security, etc.
For Self-employed: copy of complete 2016 Federal Tax return with all pages & must be notarized. Also, a certified copy of your tax return must be requested from the IRS. Certified copy of taxes can be obtained at:

For Social Security: Register for a My Social Security Account at: or call toll free: 1-800-772-1213
For unemployment: documentation of that income must be provided. (e.g. DUA check stubs, DUA printout with wage information)
For minors: Any adult receiving income for a minor must have proof of that income.
4 / Other Information / Full time students 18 years old & older need documentation from the school or college.
5 / Child Support / If you are a single parent with custody of your child, bring whatever proof you have of child support you receive. (e.g.: Copies of cancelled checks, court order, notarized letter from support source, etc.)
6 / Housing Assistance / If you receive Housing Assistance, bring your lease or a statement from the housing office. If heat is included in your rent, bring your lease.
7 / Renters / If you rent, you must bring the name, address and phone number of your landlord. It has to be included on the application.
8 / Home Owners / If you own your own home and pay a mortgage, bring in evidence of your monthly amounts for principal, interest, real estate taxes, and home owner’s insurance.
9 / Special Rent/Utility Information / **If your rent and utilities are more than your income, your application will be denied unless you have proof that someone is helping you with your bills. We have a special form to use for this situation.
