HEALTH REQUIREMENTS AS DETERMINED BY STATE CODE:

Age:To enter kindergarten in September, a child must be five years on or before December 2, 2011.

Proof of Age:Parents must validate their child's entrance by one of the following: birth certificate, baptismal certificate, hospital records, or passport. (Ed. Code 48000, 1988)

Unconditional Admission:No student shall be enrolled unless the following minimum requirements are certified by immunization records.

IMMUNIZATION REQUIREMENTS

To enter or transfer into public and private elementary and secondary schools (Grades K-12), pupils under age 18 must have:

VACCINE / REQUIRED DOSES
Polio / 4 doses at any age, but...
3 doses meet requirement for ages 4-6 years if at least one was given on or after the 4th birthday4;
3 doses meet requirement for ages 7-17 years if at least one was given on or after the 2nd birthday.4
Diphtheria, Tetanus, and Pertussis Age 6 years and under (Pertussis is required)
DTP, DTaP or any combination of DTP or DTaP with DT (tetanus and diphtheria)

*Two doses of measles-containing vaccine required. One dose of mumps and rubella-containing vaccine required; mumps vaccine is not required for children 7 years of age and older.

1The law allows permanent or temporary exemptions for medical reasons or if immunizations are contrary to the religious or personal beliefs of the parent or guardian. For medical contraindications, parents should have a signed note from a physician specifying the reason for and duration of the exemption so they can submit it to the school district. For exemptions for mumps or rubella vaccines because the child had the disease, the physician must note that they have laboratory evidence of immunity.

2For children under 7 years of age, the physician should give the parents a signed note if pertussis vaccine is contraindicated.

3MMR doses must be on or after the first birthday. Mumps is not required for those 7 years of age and older. For grades K-3 and 7-8 grade entrants, one dose must be MMR; the other dose may be any measles-containing vaccine given on or after the first birthday (MMR vaccine usually will be used).

4Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirement.

5Receipt of one dose of Tdap, DTaP, or DTP on or after the 7th birthday will satisfy this requirement.
