Instructions:
Please address the following questions in your institutional grant request. The format outlined should be followed in the preparation of your proposal. The number of pages is not to exceed those indicated in parenthesis. If you need assistance, pleasecall Page Wolf in the Professional Development Center (x2446).
Proposals must be double-spaced typewritten.
Please use the form fields on the pages below to type your application, save it, and return the completed copy electronically to Patty McAnally, secretary of the Professional Development Center, . Please save in PDF format if possible!

Departmental Support
Please have your direct supervisor (Director, Dean, etc.) email a letter of support of your project. This letter should indicate that the project has support and that all other internal funding opportunities have been pursued. Please list the source and amount of funding (see p. 7). If you are requesting release time, overload pay, or a stipend, a letter from an appropriateadministrator needs to be submitted as well.
Support letters should be emailed care of the Professional Growth Commission to and addressed to the CLC Foundation Grants & Scholarship Committee.

Describe the specific sequence of activities that will be carried out during the grant period and the implementation timetable for the project activities.
Project Personnel
List all persons who will work on this project. For each person, indicate what role he/she will fill on the project.
Impact:
a. Short-term Impact - During the period of funding, how many people will be affected by this project and what benefits will they derive from it?
b. Long-term Impact - If it is successful, how might this project affect the college, its operations, its students, and/or its personnel over the next 2-3 years? How might what is learned from this project be more broadly applied? Do you foresee any future "spinoffs" of this project?
c. Personal Impact - What do you personally expect to learn from your involvement in this project, and how will that knowledge impact your professional competency, job satisfaction, etc.?
Evaluation Plan (one page)
How will you be able to determine whether or not this project is successful? Describe the information you plan to collect which will enable you to verify the degree to which the project has met its goals and objectives.

For each type of expense, describe in detail what you need, and how much it will cost. Also, please describe how funds will be used for the complete project.(NOTE: If you are requesting release time, please indicate under Personnel an estimate of the monetary equivalent to the release time hours).

Provide a brief summary of the impact on existing college resources (additional support, personnel, space, equipment, etc.). Indicate the sources of this support.
