____ Classroom and School:This program seeks to provide resources to fund innovative and/or unique strategies that fall outside of the traditional instructional model. Applications should include applicable research and data to support assumed outcomes. Grant outcomes must have some form of measurable performance indicators. Grants may be submitted in three categories up to: $2,500, $5,000, and $10,000. Multiple grants will be awarded in each funding category.

____ Health and Fitness: The Health and Fitness Grant specifically targets innovative, research-based practices designed to help students in need to develop healthy lifestyles designed to improve overall health, reduce or prevent childhood obesity, and reduce at-risk behaviors associated. Funded in partnership with Kaiser Permanente, up to $50,000 will be awarded in increments of $5,000 or less.

____ Patriots Character and Civic Education:The Patriots Grant promotes educational programs that support the development of citizenship, knowledge of the history of the local community, patriotism and civic pride, and ethical values as a basis of good character. Funded in partnership with the Clovis Veterans Memorial District, up to $10,000 will be awarded.

____ Emotional Wellness: Emotional Wellness grants seek to provide resources to fund strategies or programs designed to support emotional and social wellness efforts that fall outside the traditional instructional model. These programs may support student, classroom or community social and emotional wellness. Funded in partnership with Community Hospitals of Central California, up to $10,000 will be awarded.

  1. Write a brief overview of the project (10 points).Describe the purpose of the grant and associated goals to be achieved.
  1. Project Need (20 points).Explain the need for this project. What funding alternatives are available for the project and/or why is alternate funding not available? What is happening with students, the school community, and/or the learning environment that creates a need for this project?
  1. Project Objective (20 points). What do you want to improve or accomplish with this project? Please provide specific examples of how the grant project is designed to impact overall student achievement/success now and/or into the future.
  1. Project Activities (20 points). What activities, actions or efforts will be conducted to achieve the project goal? What outcomes will be used to determine success?
  1. Continuing Benefit (20 points). How will the benefits of this project continue beyond the funding year?
  1. Budget/Order Form (10 points).Include ALL information needed to place the order on your behalf. (Company Name/website, vendor address, item number, description, quantity, unit cost and total cost). If ordering books, cd’s, etc. an ISBN# MUST BE INCLUDED.

APPLICATIONS SUBMITTED WTIHOUT THE REQESTED INFORMATION WILL BE CONSIDEREDINCOMPLETE AND WILL NOT BE CONSIDERED FOR A GRANT AWARD THIS YEAR.
