Dear Sirs,

We refer to the request of the qualification process of your Company by Eni S.p.A., for the commodity class listed below.

The activation of the qualification process requires that your company acknowledges Eni S.p.A.’s Code of Ethics available on our web site: according to the terms indicated in the “Compliance Declaration” form that must be sent by e-mail with the documentation listed below, within the deadline set.

If we don’t receive all requested documents or we receive only a part of them, the qualification process will be ended with negative outcome.

The commodity class for which your qualification process is being activated is the following one:

The amount to be paid in order to start the qualification process is[Amount]VAT not included.

You are kindly requested to send us by the [Date, 3 days ]:

  • A letter for the formal acceptance of the costs to be paid(see the attached form);
  • Declaration signed by your Company’s Authorised Officer about the following H&S indicators:

Within the [Date, 15 days ]you are kindly requested to provide us with the following documentation:

  • “Compliance declaration” (see the attached form)[1];
  • Company registration licence (or other similar certificate in accordance with your national law);
  • Copies of your company audited financial statements (1 copy for each of the last 3 years);
  • Declaration about all relevant references of the last 5 years, relating to the commodity object of this qualification process, specifying: the customers, a brief description and amount of works. For works performed in Joint Venture, indicate the portion carried out by your company;
  • Questionnaire: “eni_Qualification System_General Data” (here attached);
  • Technical questionnaire: “[insert the name of technical questionnaire(s) in case of characterized commodity class]”;
  • Updated Company / Group structure chart;
  • Updated Company organization chart;
  • Company brochures or any other company presentation material.
  • Technical documentation:

  1. Most relevant references;
  2. List of manufacturing/service plants;
  3. CV of your key figures involved in the commodity class related activities providing detailed information on the role, years of experience, certification/training;
  4. List of equipment with the information of the technical specifications of the tools available
  5. Personnel Management: permanent employees (number, qualification etc.); temporary employees (number, qualification, contract duration etc.); consultants/free lance (number, qualification etc.);
  6. Training program and training matrix; type(s) of training courses, content, duration and specialized training centres involved;
  7. Management and technical qualification of equipment suppliers and service sub contractors
  8. List of Operational Procedures;
  • The following HS-Q Management Systems documentation:

  1. Documentation supporting the evaluation of your Quality Management System (Certificates or: Quality Policies, Quality Organization Charts, updated Quality Manual, Index of operating Procedures / work instructions, etc…);
  2. Documentation supporting the evaluation of your Health and Safety System (Certificates or: H&S Policies, H&S Organization Charts, Safety Manual, H&S operating procedures, H&S training activities – planning and registration/certification, Risk assessment document regarding activities under evaluation etc…);
  3. Documentation supporting the evaluation of your Environmental Management System (Certificatesor: Environmental Policies, Environmental Manual, Environmental operating procedures etc.).

Once we receive your official statement concerning the qualification costs acceptance, we will deliver your company the related invoice, together with the bank account details for the payment.

Therefore, you are kindly requested to reply this e-mail address, sending a copy of the bank wire transfer notice.In case of payment failure,Eni reserves the right to terminate the qualification process with negative outcome, or to proceed with the disqualificationif the process is already closed, taking appropriate measures.

The fee is intended to cover only a part of the internal expenses Eni bears for carrying out the qualification process. The amount paid will not to be returned in any of the following cases: supplier gives up on the qualification process, supplier’s delay in providing the qualification documents, negative qualification process outcome, unqualified or not qualified status
