Form Instructions & Application Guidelines

Dear Faculty Member,
Thank you for providing an appraisal for a Biology Department Scholar. Your candid responses will greatly assist the committee in selecting talented Biology majors who deserve to receive this distinction. This form is fillable and may be saved to your computer. N.C. A&T Biology Department faculty: Please submit your completed appraisal form to the Biology Scholarship Forms ‘Group’ using your Aggie university email. Please take care to place the form in the appropriate folder (e.g. 2016-2017 folder for the 2016-2017 academic year). All other faculty members, please email to Dr. C. Dinitra White () with subject heading (Biology Scholarship Faculty Appraisal Form for STUDENT NAME).
As a reminder, each applicant should meet the following eligibility requirements in accordance with the Department of Biology:
·  Student must have a minimum 3.25 cumulative grade point average (GPA)
·  Additional criteria will include:
a.  New Freshman: Final High School GPA
b.  Continuing students: Current GPA with a focus on performance in biology, chemistry, and mathematics
c.  Funding received from sources outside of the Biology Department
d.  Residential status
e.  Total number of applicants
f.  The Biology scholarship is a need based award and also based on funding availability
·  If a student receives a scholarship award, she/he will be required to participate in the Undergraduate Student Ambassador (USA) program in which students volunteer to participate in various projects in the department of biology. These opportunities may include peer mentoring, participating in alumni events, attending the donor breakfast, serving as an ambassador for the Pre-Professional Scholars Program Professional Development Conference, etc.).
