I, the RENTOR, understand how to close the camera housing or access points properly. ______

For cameras with housings, the RENTOR understands that a leak test without the camera inside is strongly recommended before submersion with camera inside. ______

I understandthat I (RENTOR) am responsible for ANY LOSS and ALL DAMAGE, including flooding, that occurs whilst equipment is in my possession. ______

The RENTOR is responsible for all damages that occur whilst in use by said RENTOR. These damages include but are not limited to dropping, water, sand and or dust damage, scratches of housings or lenses and broken or missing pieces. Misplaced items such as housings, memory cards, chargers, leads, filter sets, batteries, frames and lights will be chargeable at full retail cost if lost or stolen.

The RENTOR hereby acknowledges receipt of the equipment designated on this form, and, if any of this equipment is to be used for scuba diving, that RENTOR is a certified scuba diver or student in a scuba diving course/ program under the supervision of a certified scuba instructor. ______

The RENTOR acknowledges that the equipment is in good working condition and that he/she has examined the equipment to ensure that it is free from defects, and that the user knows how to properly work the equipment. ______

The RENTOR understands that this equipment is being rented to the above named individual. The RENTOR accepts liability for any damage caused to the equipment during the above defined rental period, regardless of who causes the damage. If the RENTOR decides to lend any equipment or accessories to another guest on board the vessel, the RENTOR will be still be liable for any damages or replacements. ______

The RENTOR is either able to use their own personal memory card or they can purchase a 32GB Micro SD Card from blue o two for use within their equipment. This memory card then becomes property of the RENTOR to take home with them at the end of the trip. The RENTOR also understands that blue o two and its employees, owners, officers, or agents (hereinafter “Released Parties”), shall not be held liable or responsible in any way for any injury, death or other damages to RENTOR or his/her family, heirs, or assigns which may occur as a result of the rental and/or use of the equipment, or as a result of product defect, or the negligence of any party, including the Released Parties, whether passive or active. ______

I, ______(Rentor’sname), HAVE CAREFULLY READ AND UNDERSTAND THE ABOVE AGREEMENT. BY SIGNING THIS AGREEMENT, I EXEMPT AND RELEASE BLUE O TWO AND ALL RELATED ENTITIES AS DEFINED ABOVE, FROM ALL LIABILITY OR RESPONSIBILITY WHATSOEVER FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH AS A RESULT OF RENTING AND/OR USING THIS EQUIPMENT, HOWEVER CAUSED, INCLUDING, BUT NOT LIMITED TO PRODUCT LIABILITY OR THE NEGLIGENCE OF THE RELEASED PARTIES, WHETHER PASSIVE OR ACTIVE.
