CYO ATHLETICS

NON-TERRITORIAL PARISHONER FORM

2016-2017

For Players Post - Confirmation

My son/daughter received the Sacrament of Confirmation at ______

Parish, in the City/Town of ______.

Since “parish” is a spiritual entity and membership does not rest solely with geography, the CYO program offers the NTP opportunity for youth to participate. However, it is important to remember youth are not allowed to play for a team if it is their only connection with the parish. This would detract from the sense of fair play among all parishes of the Archdiocese.

1. Non Territorial Parishioner (NTP) If any youth does not reside geographically within the parish boundary, as defined by the Archdiocese of Boston, but whose family is registered with the parish, attends Holy Mass, and is active in that parish he/she may play ONLY if the parent/guardian completes the Non-Territorial Parishioner (NTP) form and submits it to Parish Athletic Director and the Pastor of the parish in which they are registered. If approved by the pastor it is to be submitted to the Area Coordinator by November 15 for approval.

I, ______parent / guardian of______

attest to the fact that ______would like to play on the

(Parish)______(City/Town)______basketball team this season does not live within the geographic boundaries but, with his/her family is indeed a parishioner here because of his/her involvement in this parish which includes attending Holy Mass. [Please attach a letter addressed to the pastor indicating how he/she is involved in the life of the parish (e.g., Faith Formation Program, membership in parish organizations, altar server, lector, choir, programs of sacramental preparation, family activities, etc.) and any pastoral situations that should be considered].
