1.  *Identifies the tasks that are high risk construction activities (as per WH&S Regs 2011 S 299) See QUT Guide for Jobs Needing SWMS.
2.  *SWMS is relevant to this job as appropriate– Maybe generic but SWMS must consider the hazards specific to this job and circumstances or the PCs Construction Safety Plan for which it has been developed. Separate risk assessments may be attached as necessary.
3.  *SWMS is current / dated and authorised appropriately ( document control preferred)
4.  *SWMS clearly states all information in a way that is easily understood to all relevant persons concerned.
5.  *Defines all the relevant hazards & associated risks for this work/job
6.  *Description of how the work is intended to be done (method) and the relevant safety risk controls to be used.
7.  *Describes how the effectiveness of those controls will be checked and monitored by the contractor e.g.: daily checks, work permits; inspections
8.  *If hazardous tasks require specified training or competency, this is clearly defined in the SWMS– e.g.: EWPs ,materials or personnel hoists, earth moving machinery, forklifts, scaffolding OR

Safe Work Method Statement Check

For High Risk Construction Work

In compliance with the Qld WH&S Regs 2011

§  This safety plan has been reviewed by QUT for “the provision of information” as required by WH & S Regs S 299, for all High Risk Construction work or other hazardous tasks as requested.

§  It does not constitute “approval” of the work method statement.

§  Refer to QUT Guide to High Risk Construction Activity requiring a Safe Work Method Statement (G805_028) for more information about which jobs require SWMS.

§  Work must not be allowed to start unless an authorised University representative [ project co-ordinator /other authorised officer] of QUT University has:

o  Discussed the relevant aspects of this safety plan with the contractor and

o  Is satisfied that the contractor understands and is capable and willing to comply with the work method statement, as they have submitted.

§  Safe Work Method Statements must be amended after any significant changes in activity method, and an amended copy provided to University project co-ordinator /officer as soon as possible.

§  Safe Work Method Statements are to be available for inspection at any time during the course of the work/job.

§  Responsibility for implementation of this safety plans rests with the contractor submitting the plan.

§  Where work is not carried out in accordance with the SWMS , QUT or the appointed Principal contractor may request that the work or project cease until the relevant safety issues are corrected.
