THE GRASSHOPPERS PROGRAM IS A SCHOOL-YEAR PROGRAM BEGINNING MONDAY, SEPTEMBER 11, 2017 AND ENDING THURSDAY, JUNE 14, 2018.
YOU MAY CHOOSE ONE (1) to FIVE (5) DAYS OF ENROLLMENT; PROGRAM HOURS ARE 9:00 A.M. TO 12:00 P.M.
Tuition is based on which days of the week your child attends.
IF YOUR CHILD ATTENDS ON A TUESDAY, WEDNESDAY OR THURSDAY, THE ANNUAL TUITION IS $1,930 PER DAY.
IF YOUR CHILD ATTENDS ON A MONDAY OR A FRIDAY, THE ANNUAL TUITION IS $1,880 PER DAY.

Upon your child’s acceptance into the MMO GRASSHOPPERS PROGRAM, a contract will be issued. Your child’s space may ONLY be secured upon submission of the contract and NON-REFUNDABLE DEPOSIT OF $500.00. The remaining tuition after your deposit has been deducted will be billed in ten (10) equal monthly installments.

In order for your application to be considered, you must:
· Return this form filled out in its entirety
· Submit an $85 NON-REFUNDABLE processing fee.
MMO Programs reserves the right to place children in appropriate class groupings, with regard to age and gender balance.
Students are required to be fully immunized to attend MMO, unless a medical contraindication exists. As a center affiliated with a religious institution, MMO is not required to allow religious or philosophical exemptions.
Deadline for priority registration (Church members, returning children and their siblings) is January 20, 2017. Priority and open enrollment applications will be considered in the order they are received. Waitlisted applications will remain valid through June 14, 2018.
