For all Student-Athletes,

In October of 2016, Bates College adopted a new process for Sports Participation and how our athletes will be medically cleared for sport. We felt as a group that this process would ensure our athletes would be safe to participate, provide uniformity and continuity of the Pre- Participation Exam (PPE), and bring Bates College more in line with NCAA recommendations. The PPE covers a variety of areas such as cardiac, respiratory, mental health screening, and previous and current orthopedic injuries/issues.

No longer will your home physician or an Urgent Care Center be able to clear you for sport. In order to participate in your sport, you must undertake the Central Maine Sports Medicine PPE.

You will need to sign up for an appointment on the google doc that has been forwarded to you by your coach. Please show up for your PPE 15 minutes prior to your scheduled appointment time for registration and processing. If you do not sign up for a time and show up there is no guarantee that you will be able to be seen by a physician for your PPE. In advance, please download the Pre-Participation Exam Form found at the Bates Sports Medicine webpage (http://athletics.bates.edu/sports-medicine/medical-forms) and complete prior to your exam. Bring the completed form with you to your PPE.

For the following sports: Men’s/Women’s Swimming and Diving, Men’s/Women’s Squash, Men’s/Women’s Basketball, Nordic Skiing, Alpine Skiing, Men’s/Women’s Indoor and Outdoor Track, Men’s and Women’s Ice Hockey, Men’s Lacrosse, Women’s Lacrosse, Baseball, and Softball. Your PPE will be from 8a-5pFriday, September 15that the Gray Cage.

First Year Student-Athletes: Please download and complete the following forms from the Sports Medicine webpage: Sickle Cell, HIPAA, Emergency, Concussion/Injury, Assumption of Risk and ADHD form, if applicable. Please bring these completed forms, along with your completed PPE Form to your PPE for processing and collection.

Returning Student-Athletes: Please download and complete the following forms from the Sports Medicine webpage: HIPAA, Emergency, Concussion/Injury, Assumption of Risk and ADHD form, if applicable. Please bring these completed forms, along with your completed PPE Form to your PPE for processing and collection.

Lastly,You must bring $40.00 with you to the exam. Cash, check, and credit card are all accepted.Your insurance will not be charged for this exam. This will cover the complete cost.

You should wear comfortable clothing (e.g. athletic wear) for the exam. Physicians from Central Maine Sports Medicine will provide the exam.
