As a parent/guardian of ______, I hereby give my consent and permission for the above named child to participate in the EAWR JFL Football and Cheerleading Organization. The program will be conducted under the EAWR JFL Football and Cheerleading Rules and Authority. I, the parent agree to abide by all the rules and requirements set forth by the EAWR JFL Football and Cheerleading Organization. I further agree to accept full responsibility for all expenses incurred for the loss of any equipment issued and not returned in good condition at the conclusion of the season. If I refuse to return any equipment or pay any expenses incurred, the EAWR JFL Football and Cheerleading Organization can and will prosecute to the fullest extent of the law.

In addition, I, the undersigned, verify that the above information is correct and also understand that the insurance provided by the EAWR JFL Football and Cheerleading Organization is a secondary type of insurance with the primary insurance being that of the parent or guardian.

The undersigned states that he/she understands that the EAWR JFL Football and Cheerleading Organization is not and shall not be responsible for or liable for any illness, or injury to person or damage to property resulting from the program in which the undersigned is enrolling or being enrolled or from his/her participation in said program and the participant and the undersigned, if the participant is a minor or under other legal disability, hereby forever releases and holds harmless the said EAWR JFL Football and Cheerleading Organization its employees agents and representatives from any and all claims of any kind that the participant or the undersigned or their respective heirs, executors, administrators, or assign may have or claim to have resulting from participation in said program. ABOUT THE EAWR JFL

1. What is the JFL?

The JFL stands for (Junior Football League). The JFL provides children in the 5th through 8th grade the opportunity to participate in tackle football and Cheerleading. We are part of the South Central JFL Conference. The conference has teams from Greenville, Hillsboro, Litchfield, Vandalia, Carlinville, Gillespie, Staunton, Pana, Southwestern and Roxana.

2. Who can play? Participation? What are the ages and weights?

Football and Cheerleading participants in the 5th through 8th grade only. For: (8th grade- cannot turn 15 before Sept, 1 2011) (7th grade- cannot turn 14 before Sept, 1 2011) (6th grade- cannot turn 13 before Sept, 1 2011) (5th grade- cannot turn 12 before Sept, 1 2011). There are no weight restrictions to play football or cheerleading. There are restriction to run, catch and pass the football. (8th grade 155lbs)(7th grade 140lbs)(6th grade 125lbs)(5th grade 110lbs). Note: Depending on the roster size, some circumstances may require a player to participate up or down one grade level.

3. When is registration?

You have three sessions dates to register, but only need to pick one. Please reference flyer for information.

4. How much does it cost to participate?

The registration fee for both football and cheerleading is $65 if you sign up early. If you sign up late, the cost is $85. Registration dates are provided on the flyer. In addition to the registration cost, the parents and players will be asked to help support the organization by selling a few fundraising items to help with start up cost.

5. What is needed to participate?

Participants will need to complete the Registration Form and Parent/Player Code of Conduct, provide payment, provide a copy of his/her most recent physical, and purchase cleats and jock .

6. Is this JFL associated with the Middle Schools?

No, The JFL is not associated with either middle school. This is a club team, organized and funded through the parents and players. The middle schools have no decision making authority. Please do not contact the middle schools with regard to JFL issues or policies.

7. When are Practices scheduled?

The first JFL practice for all grade levels will be July, 18th 2011. Practice for all levels will begin around 5:30 in the evening at the high school practice field. For the first 4 weeks, practice will typically be held Monday-Thursday. It is at the coaches discretion to practice more than 4 days per week but the minimum should be at least 4 days through out the week. Once the games start, practice will typically be held Tuesday & Thursday 2 days per week. Once again, it is at the coaches discretion to practice more than 2 days per week, but the minimum should be at least 2 days. Once your coach is identified, please contact him or her with practice dates and times. With most of the coaches working day jobs they will have the flexibility to make changes accordingly. Practices will take place at the high school practice field.

8. When are Games scheduled?

All JFL Games are played on Saturdays whether home or away. First game is scheduled for August 13, 2011. 5th Grade plays @ 12Noon, 6th Grade plays @ 1:30PM, 7th Grade plays @ 3:00PM and 8th Grade plays @ 5:00PM. Players must arrive at least one-half hour before game time. Please discuss this with your coach, he or she may have you arrive earlier. If games are rained out on Saturday's, the games are automatically re-scheduled for Sunday. All home games will be played at the high school

9. Will there be Bus transportation for the players?

No. The parents are responsible for all transportation to and from practice and games. Bus transportation will not be provided.

10. Do I need to provide proof of medical insurance?

Yes, This is covered under your registration form. The club will provide you with a form of secondary insurance but understand that the primary insurance will come from the parent and this will be used first. Insurance can come in the form of medical card, state aid, or company insurance.

11. Do I need to purchase Football and Cheerleading equipment or pay a deposit?

No. A full uniform will be provided for football with the exception of cleats, socks, and jock. The parents will need to provide for this. For cheerleading, a top, bottom and pom's will be provided. Accessories such as: Hair Duties, Sock's and shoes will not be provided.

12. How do I get involved with volunteering my time and helping out?.

With this being a totally club team, parent participation is a must. Without participation and cooperation by the parents, this program will not work.
