Food Service Management Company (FSMC) Application for State Agency Approval

Thank you for your interest in obtaining approval from the Georgia Department of Education to serve and/or provide meals to school food authorities (SFAs) as a part of the National School Lunch Program.

Please read the following application in its entirety and provide all requested information where indicated. Any incomplete applications will automatically be rejected.

Applicant responses will be evaluated by the Finance and Business Unit of the School Nutrition Program and weighted according to the following categories:

Company History : 10 points

Financial Stability: 18 points

Personnel and Training: 20 points

Food Service Experience 10 points

Past Performance and/or References: 24 points

Experience Providing Meals through the National School Lunch Program (NSLP): 18 points

The maximum amount of points you may earn is 100. Any applicant that does not achieve 75 points or higher will be asked to reapply and will not be approved.

If approved, your FSMC must agree to strictly adhere to federal regulations set forth by USDA as a participant in the NSLP; failure to comply with any federal or state regulations may result in suspension or termination of approval of your FSMC and your company may be omitted from the approved vendor/caterer list for the state of Georgia for a minimum of one year or permanently removed.

In order to demonstrate character, production capability and financial stability, each FSMC is to provide, a complete company history, overview and financial background report. This report is to include:

A.Name and address of company or firm and its officers or operators, and a brief biographical sketch of the key food service personnel to establish their qualifications in the field. Please include designated point of contact for Georgia Department of Education, along with email address and phone number.

B.A description of FSMC's plant site, size (square footage), and production capability.

C.Permission for the representatives of the Georgia Department of Education’s School Nutrition Program to inspect the contractor's present facilities at any time during the duration of a contract with the School Food Authority.

D.Ethics: Are you aware of any interest or potential interest in your company or this contract that may be held by an individual employed by the Georgia Department of Education

Alsoprovide details of how much relevant experience you possess, including what school food authorities (SFAs) you have worked with and the capacity in which you supported SFAs through the NSLP.
