Follow-up Survey Data Summary, Cycle 14 October 14, 2013
A pool of 565 individuals for whom IM4Q had both a completed EDE for FY2012-13 Cycle 14 and one or more considerations with Supports Coordinator responses was generated to complete the follow-up survey. A goal was set to contact 10% of this pool for a follow-up survey. Respondents ranged from residents in community homesto those in family residences to those in nursing homes.
Out of the pool of565, a representative subset of 150 potential interviewees was generated. A total of 56individuals (or their representatives)were surveyed regarding a total of 90 considerations. Follow-up surveys were conductedvia phone and in personbetween September 3-25, 2013.
Reporting, Action, and Satisfaction:
After sharing the consideration and response with the interviewee, they were informed that “this consideration was listed by the interviewer as a concern”. Interviewees were then asked, “Do you remember telling us this during the IM4Q interview?” 75considerations (83%) were reported by interviewees as they “remembered telling us (team) about this concern”.
Interviewees were then asked, “Did your Supports Coordinator discuss this issue with you after the IM4Q interview?”Respondents stated that of the 90 considerations surveyed, 77(86%) were reported as being discussed with their Supports Coordinator after the consideration was entered by the IM4Q team.
Interviewees were then asked, “Has any action been taken to address this consideration?” Respondents stated that of the 90 considerations surveyed, 67considerations(75%) were reported by interviewees that action had been taken to resolve or address the reported issue.
Interviewees were then asked, “Did you get what you wanted?” Respondents stated that of the 90 considerations surveyed, 74considerations (82%) were reported by interviewees as they “have gotten what they wanted” from the actions and outcomes regarding the original issue reported by the IM4Q team as a consideration.
Interviewees were then asked, “How happy are you with the action taken to address this consideration?” Interviewees were asked about the satisfaction of the outcomes for each individual consideration reported and addressed. This question was answered on a 1-10 scale, with 1 representing Very Unhappy and 10 representing Very Happy.
Living Arrangement Overview: The 56 respondents live in9different living arrangements.
Considerations Overview: The 56respondents were asked about 90 considerations in total. The considerations that were addressed in the follow-up came from 11 different consideration theme categories.

When we spoke about emergency preparedness, XXXXXXXXX mentioned that she borrowed a flashlight during Hurricane Sandy, but does not have her own. Can she be assisted to acquire emergency kit supplies?
