Florida 4-H would like to invite you to take part in an evaluation about what you have learned in 4-H this past year. In this study, we want to know what you gained from your 4-H program experience specifically in the areas of citizenship, communications and getting ready for the world of work. Your parents have been informed of your possible involvement in this evaluation.

If youdo not want to fill out the survey that is okay or if you want to skip questions that is alright as well. If you decide to participate in the survey, you can stop at any time. Although we hope you will complete it!

It will take you about 20 minutes to complete the survey. If you have any questions at any time, please ask one of our leaders or your county 4-H agent if available.

Your county 4-H agent may send out a message or two reminding you about completing the survey through Facebook, email, Instagram or Twitter. These are just reminders. The survey will close on September 15.
