PROJECT ACCOUNTABILITY REPORT FORM
A Project Grant Accountability Report must be completed by the organization responsible for the project or by the project’s administrative staff within 30 days of the end of the project’s first year of funding and every year thereafter that the project utilizes FCF awarded funds, as well as at the termination of the project.

II. NARRATIVE STATEMENTS: (Maximum 4 pages)
- Accomplishments
Describe the project’s goals and the success or lack of success in meeting goals since the last project report or, if this is the first report, since funds were awarded. Indicate whether the goals, target population and implementation strategies have changed over the past year. Explain how the target population was affected by the project. If a revolving loan system is used, describe the effectiveness of its application, dispersal and repayment processes.

- Evaluation
Describe how the effectiveness of the project is measured, any unexpected results encountered during the reporting period, what was learned, and what changes are planned for the next year.
- Administration
Describe any significant changes in the organization and in the project, if any, including changes in objectives, operations and staff. Provide brief bio-resumes for any administrative staff new to the project during the reporting period.
III. FINANCIAL INFORMATION:
- List in detail the actual project income and expenditures over the reporting period and explain any significant changes or major discrepancies from the original approved budget for the year.
- Identify the individual sources of project income and the amount provided by each source over the reporting period.

- Attach the most recent financial statement or annual report of the organization, if not already submitted. If already submitted, please indicate when the next financial statement or annual report will be available.
- Optionally, you may attach any relevant materials that might help describe the project and its progress, e.g. brochures or other publications, newspaper articles, website addresses, videotapes, annotated photographs, participant testimonies, etc.
