What is PASRR? Preadmission screening and resident review (PASRR) is a required screening of all individuals who are being referred to Medicaid-certified nursing facilities (NF) and who may have a serious mental illness (SMI) or an intellectual disability or related condition (ID/RC). As a condition of approval of the Medicaid state plan, each state is required operate a PASRR program.
What rules apply to PASRR? PASRR is governed by 42 CFR §483.100 - §483.138 and WAC 388-97-1910 – 388-97-2000.
What does PASRR do? The PASRR Level One identifies individuals who may have a SMI or ID/RC and notifies the Regional DDA PASRR Coordinator or the Mental Health PASRR Evaluator, or both, if the individual is believed to have both SMI and ID/RC. The PASRR Level Two confirms the presence or absence of SMI or ID/RC, determines whether NF care is appropriate, and determines whether specialized services for SMI or ID/RC are needed, in addition to NF care.
Who completes the Level One? Level One is completed by the referring party, usually the referring hospital or physician.
Where can I find instructions for completing the Level One? Form instructions are on the last page of the Level One form.
When should the Level One be completed? The Level One should be completed as soon as NF placement is considered and there is reason to believe the individual has SMI or ID/RC. Indicators of SMI or ID/RC require follow up by the mental health (MH) PASRR evaluator or the ID/RC PASRR evaluator, and the follow-up must occur prior to NF admission.
What do I do with the completed Level One? Forward the completed Level One to the MH PASRR evaluator or the ID/RC PASRR coordinator immediately if either ID/RC or SMI is indicated. If both ID/RC and SMI indicators are present, two referrals must be sent. See Contact Info at the links below. Additionally, a copy of the completed Level One should be sent to the receiving NF.
