Family Needs Assessment 1302.50(a)(b)(3), 1302.52(a-c)

Policy

KI BOIS Head Start will implement a family needs assessmentto identify family strengths, needs and goal priorities related to the family engagement outcomes as described in the Head Start PFCE Framework. These outcomes include family well-being, parent-child relationships, families as lifelong educators, families as learners, family engagement in transitions, family connections to peers and the local community, and families as advocates and leaders. Identified family needs will be addressed through support, information and referrals.

Procedure

  1. Based oninformation gathered fromthe Family Needs Assessment (FNA)(KHS-820), individual partnership agreements may be written to reflect goals, responsibilities, time lines and strategies for achieving the goals and monitoring progress.
  2. The Family Service Worker (FSW) will be responsible for assisting the family with completion of the FNA, at a home visit or parent staff conference,no later than 60 calendar days from the date of enrollment.
  3. Family Service Workers may accompany center staff for the enrollment home visit to complete the FNA when possible.
  4. The FNA form is a questionnaire that will serve as a conversational guide to engage families in discussion to determine what they perceive as their needs and strengths.
  5. In areas where the family has identified needs, the FSW will make appropriate referrals and provide resource information and materials.
  6. The Family Needs Assessment, Family Partnership Agreement and Family Education Survey will be completed at the same time when possible.
  7. Once the FNA has been initiallycompleted with the family, the FSW will continue to work with the family to see if additional resources and/or referrals are needed to meet their needs. This will be completed by the FSW updating the FNA with the family again at the end of the year.
  8. Parents of dual language learners will have a translated note/letter/information, or an interpreted call, staff conference, or home visit whenever possible.
  9. The Family Service Worker will document as follows:
  10. Completion of the Family Needs Assessment form in child’s file and pre and post data entered intoChildPlus. This information will be aggregated, analyzed and shared.
  • Document home visits or parent staff conferences and additional contacts in child’s

file and/or ChildPlus as necessary.

  • Note any referrals and resources given to the family in child’s file and on the Family Engagement Tracking Report action page (KHS-813).
  • Document any follow up that needs to be done/has been completed on Family

Engagement Tracking report and wherever necessary in child’s file.
