Wellness Coordinator

Reports To: Clinical Manager

Supervises: Health Coach

Summary:The Wellness Coordinator is responsible for all programs designedto promote optimal patient health outside of clinic visits.

Primary Duties:

The Wellness Coordinator has primary responsibility for the following programs:

  • Celebremos La Vida Program:

This program is a breast and cervical cancer screening program targeting Hispanic women over 40 which operates once a month at Centegra Woodstock Hospital. It is a full day of screening one on one health education. The Coordinator schedules patients, organizes day’s activities, plan lunch for staff and volunteers. Additionally, the Coordinator provides follow up and navigation services (including interpretation) for those needing additional services. Data collection and reporting is critical.(monthly program)

  • CareMessage Program:

This program is an automated health education texting program designed to address several chronic conditions. The Coordinator recruits patients, sets up the programand monitors responses. Coordinator also analyzes data and compiles reports (daily/weekly program)

  • Group Visits:

In conjunction with the nurses, recruits patients for this 6 month program (on select chronic disease topics). Coordinator plans activities, confirmspatients and volunteers,and assist during group visit (monthly program)

  • DEAR Program:

The Dear Clinic is a collaborative autism diagnostic clinic. The Coordinatorworks only with the clinical portion of the program andprepares charts, rooms patient, obtains vital signs, interprets for provider (monthly program)

Secondary Duties:

  • Supervises and provides guidance to Health Coach
  • Provides community presentations on selected topics
  • Helps develop resources to enhance wellness offerings
  • Assist volunteers with their assigned roles and serve as a resource for them
  • Serves on community boards/coalitions when appropriate
  • Seeks out collaborative projects with other community partners
  • Deliver presentations to FHPC Board and at select conferences when appropriate

Skills and Desired Background:

  • Fluent in English and Spanish, both oral and written
  • Degree in a health related field or background in a health care field is preferred
  • Strong internal motivation
  • Self-directed with solid critical thinking skills
  • Detail oriented while remaining flexible and collaborative
  • Strong computer skills and familiarity with electronic medical records preferred
  • Proficient in obtaining vital signs, especially blood pressure
  • Effective oral and written communication skills
