The following agreements, releases, and waivers pertain to persons participating in Studio Seven Center for Creative Studies’ classes and other activities. SSCCS is interchangeable with Studio Seven Center for Creative Studies.

ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY

I hereby consent to the aforementioned person participating in Studio Seven Center for Creative Studies’ classes and activities. I acknowledge that potentially severe injuries may occur in any activity involving motion, including dance, tumbling, acrobatics, and other related activities. In consideration for allowing the above student to utilize SSCCS’ facilities and for other good and valuable consideration the receipt and sufficiency for which is acknowledged. I hereby forever release, discharge and hold harmless (“SSCCS”), Studio Seven LLC, its owners, shareholders, members, managers, directors, officers, employees, independent contractors and instructors from any and all liability for any and all damages and injuries suffered by the above student, while under the supervision, instruction, or control of SSCCS and/or that relate to in any way to the student’s participation in any activities offered by SSCCS . As legal guardian of the above mentioned student, I hereby agree to individually provide for the possible future medical expenses, which may be incurred by the above student as a result of any injury sustained while training at or performing for SSCCS. This acknowledgment of risk and waiver of liability having been read thoroughly and understood completely is signed voluntarily to its content and intent.

APPEARANCE RELEASE

Condition of Participation: Parent/legal guardian signature is required in order for minor (student) to participate in classes, dance concert, and other activities.

I understand that as a participant in classes, in the annual dance concert, rehearsals, performances, and/or any other activities, I may be or the minor (student) may be included in recordings or photographs taken during these activities.

This may include, but not limited to, a class photograph and/or a video recording of the concert to be used for sale by Studio Seven Center for Creative Studies or its assignees.

The minor’s (student’s) face, likeness, voice, and appearance as a part of these activities may be used in advertising and promoting Studio Seven Center for Creative Studies.

Therefore, without reservation or limitations, I, on my own behalf and on behalf of the minor (student), hereby assign, transfer, and grant to Studio Seven Center for Creative Studies, its successors, assignees and all other commercial exhibitors the right to photograph and/or the exclusive right to videotape the minor.

Studio Seven Center for Creative Studies

Policies 2014-2015

This form represents the agreement between the student______and caregiver______and Studio Seven LLC. Your signature indicates that you are in agreement with, and will abide by, all policies and terms set forth below. A separate form is required for each enrolled student.

  1. Registration/Tuition:There will be a $20 registration fee (each additional family member is $5) assessed at the time of enrollment. This fee reserves the student’s place in class, and is non-refundable. If a student is ONLY enrolling in Kindermusik, Art or PRIVATE music lessons, the registration fee is $10, with each additional family member being $5. Current tuition rates are posted on our website. Tuition is a 45-week fee divided into 10equal monthly payments (August-May). The tuition is determined by the amount of classes taken by the entire family, not each individual student. Please note, tuition fees are due on the FIRST lesson of every month, and are the same, whether or not there are 3, 4 or 5 lessons in any given month. We DO NOT prorate tuition, or provide refunds for classes missed. We will, however, allow the student to attend a make-up lesson in a like class. If you decide to drop a class, please provide a two-week notice. If payment is not received by the 10th of each month, a $10 late fee will be added to the account.
  2. Calendar:Please see the calendar of events on our website ( for all important dates and events. If there is a reason that the studio needs to close with little warning, all efforts will be made to contact the parent/caregiver via cellular phone or email. It will then be posted on our website, followed by an update on our Facebook page ( studiosevenflorida.com).
  3. Payment:Studio Seven Center for Creative Studies takes payment via cash, check or credit/debit card. Checks may be made payable to Studio Seven, and a $25 fee will be assessed for all returned checks. Receipts are available from the receptionist, or may be e-mailed to you. To avoid potential late fees, it is suggested that you set up an automatic payment, should your bank offer this service. The tuition fee is NOT inclusive of dance apparel, footwear, costume costs, accessories, recital fees or tickets; these are additional expenses.
  4. Attire:It is essential for students to be in proper dance attire when participating. Please see the class description page on our website for specific requirements for each class. Acceptable clothing includes: tights, leotards, dance skirts, dance pants, leggings, and non-baggy t-shirts and sweatpants. If the student is wearing a leotard, shorts, and/or a skirt, dance tights arerequired. Bare midriffs are NOT allowed. Hair must be pulled back and completely off of the face (for ballet, a bun is required). Aside from small stud earrings, no jewelry is permitted while dancing. Studio Seven Center for Creative Studies is NOT responsible for lost or stolen belongings, so please ensure that personal items are left at home. Proper footwear is critical both for technique, and in the prevention of injury. Suggested footwear and tights are available for purchase in studio. Satin slippers are NOT approved footwear.
  5. Pick-up/Drop-off Procedures:Please ensure your student is dropped off with adequate time for drinks, restroom use, and for putting shoes on, as to minimize classroom disruption. Please also ensure that your student is picked up within 15 minutes of the end of his or her class. Students are required to stay inside the building until an authorized adult (as indicated on Authorized Pick-up/Drop-off form) comes inside to take them home. For student safety and traffic congestion purposes, it is REQUIRED that you park your car in a space, and come in to pick up your student.
  6. Classroom Observation:Parents are not permitted inside of the dance room unless invited by the instructor. This helps the instructor to maintain the complete attention of the student, and negate any distraction that may be caused by family members and friends being in attendance. We do have observation windows, but please refrain from gathering at the window for extended periods of time. We understand the importance of seeing the progress that your student is making, so official observation days will be scheduled throughout the year. Please see the calendar of events on our website. If you need to speak with an instructor, please see the receptionist or Ms. Casey to schedule an appointment.

$50 permonth (includes required digital home materials). If one or more siblings enroll, a 50% discount will be given to each additional child.

Barre Challenge/Zumba

$7.00 per class, or purchase a class card ($60 for ten classes). Class cards expire three months from date of purchase.

RECITAL/SHOWCASE: We will have two performances this year. A semi-formal winter showcase will be held in mid-December, and our end-of-year production will be held at Charlotte Performing Arts Center on Saturday, May 30th, 2015, with a mandatory technical/dress rehearsal on Friday, May 29th. Students are NOT required to perform in either show, but please notify us with ample time if your student will not be participating. We will not be ordering costumes for our winter showcase, but will be doing so for our end-of-year production. The cost is $75 per costume; a $25 deposit per costume is due on November 1, 2014, and the remaining $50 per costume is due on January 15, 2014. The costume fee in not inclusive of shoes and tights. There will be a recital fee of $40 per child that includes two show tickets and a performance t-shirt, and is due on March 15, 2015.
