  1. What is the name of the order that “triggers” the F2F documentation alert for physicians?

We built the same triggers for pediatrics as adult. The following orders in the CareConnection order catalog trigger the F2F alerts for physicians:

  • Discharge Planning Eval Adult
  • Discharge Planning Eval Pediatric/Mother-Baby
  • Social Work Referral Adult
  • Social Work Referral Pediatric/Mother-Baby
  1. On the Discharge Planning Eval order, what happens if you don’t select Home Health Evaluation but another choice like “Other.”

Choosing Home Health Evaluation is what triggers the rule process in CareConnection. If a different choice like “Other” is selected, it will not start the rule process (ie., Alerts for Physicians). Selecting Home Health Evaluation is the standard workflow for ordering Home Health and should be utilized.

  1. If the hospitalist has already completed a general progress note on the patient and they go to do the F2F PowerNote – after they fill out the top section with HH info, can they just say refer to progress note where all the other information is located?

That is acceptable documentation and we can print the progress note off, along with the F2F form and scan into OnBase.

  1. Does the F2F alert for physicians trigger only for Medicare and Medicaid patients?

No, currently it fires for all payers.

  1. Some physicians are still completing the old progress note - it doesn't appear to be turned off in CareConnection?

The old PowerNote has been removed from PowerNote. However, some physicians have saved it to their favorites and they have to delete it manually. There is a CareConnection report that can be run from the -> Explorer Menu -> PowerNote Usage Reports -> PowerNote HomeHealth/Oxygen Report. Those supporting on-site can reach out the docs showing up on the report and work with them on the new F2F documentation and deleting the old PowerNote.

  1. What happens if an order is placed for Home Care Evaluation and then the patient ends up going to a SNF? Will the physician continue to get an alert?

The original home care evaluation order needs to be discontinued in order for the alerts to stop firing for the physician. The alerts will stop once the patient is discharged.

  1. Where do I print the F2F Progress Note and Certification Statement from in CareConnection?

Go to Reports & Summaries -> Clinical Documentation Folder -> Physician Documentation -> Home Health Certification. Once completed, both the Progress Note (appears as Home Health: Face to Face – AHC) and Certification Statement (appears as Home Health Face to Face Form) appear in this folder. You can double click to open the document and click on the Printer icon to print.

  1. What do the DO and DPM credentials stand for next to the signature line on the Certification Statement?

DO is Doctor of Osteopathic Medicine and DPM is Doctor of Podiatric Medicine.

  1. Why can’t an APN or PA access and chart on the F2F Certification Statement?

CMS only allows a physician to complete and sign the F2F Certification Statement, as a result, APN’s and PA’s do not have access to chart on these items in CareConnection.

  1. For an MD office referral, if a patient had an MD visit a month or two ago, and today calls in and speaks to the physician and he/she decides to order home health services for the patient, can that be documented as a F2F Encounter (progress note)?

No, CMS expects that the physician, APN or PA have a face-to-face encounter with the patient (not a phone conversation) to assess the need for home care. The physician would need to have the patient come in to the office to be assessed, then complete the F2F Progress Note and Certification Statement.

  1. What happens if a physician is unable to complete the progress notes with all the required elements and/or the certification statement prior to the patient’s discharge?

The physician order and F2F documentation (progress note and certification statement) must be complete and attached to the home health referral or the referral will not be accepted by Advocate at Home.

  1. Can a resident complete a progress note and certification statement?

YES, residents can complete the F2F encounter progress note, however, they are not allowed to complete the Certification Statement because they are not Medicare certified.
