2008EXCELLENCE IN SERVICE AWARD

PRESENTED BY UCLA STAFF ASSEMBLY

To recognize, encourage, and reinforce the performance of staff members who go above and beyond the call of duty in helping the University achieve its mission, UCLA Staff Assembly is proud to announce the Excellence in Service Award. The awardwill be presented to a full time UCLA staff member who demonstrates a commitment to the mission of the University through significant contributions and achievements which consistently exceed the established requirements of the individual's appointed job. Self-nominations are not accepted for this award. If members of your department are submitting multiple nomination letters on behalf of one individual, please identify one contact person on the nomination form and attach all letters of support to the form. The individual selected for this prestigious honor will receive a $5,000 award.

ELIGIBILITY

  • Any UCLA career, full-time limited, or full-time contract employee with at least five years of employment may be nominated to receive the Excellence in Service Award.

  • Students, faculty, current Staff Assembly Executive Board members, and previous recipients of the award are not eligible.

  • Nominees must have sustained a record of excellence during their employment, and demonstrated outstanding citizenship in the University and the community.

CRITERIA

The following criteria are considered in the selection process:

  • Career of service reflecting a strong commitment to the University's mission of teaching, research, community service, and patient care.

  • Initiative, Innovation & Creativity

  • Visionary Leadership

  • Community support and citizenship

  • Focus on customer satisfaction

  • Significant contributions to the achievement of broad-based organizational goals, such as work process improvements, enhancement of teamwork, business results, etc.

PROCESS

Nominations may be submitted by faculty, staff, or students. The Award Committee will verify eligibility of all nominees.

All nominations must include a letter of recommendation from the nominee’s direct supervisor.

The head of the nominee’s department must sign all nomination forms.

Supporting letters for the individual nominated are welcomed from peers or managers outside of the nominee’s department.

Nominations must be received no later than May 5, 2008.
