Evidence of Professional responsibilities
The Application Verification page will be signed by the Superintendent, the principal, and two other reviewers to be determined by the Master Teacher Local System Coordinatorto confirm the teacher’s implementation of the following professional practices. By signing the Application Verification, the Local System Review Team is vouching that the applicant is a viable candidate for the Master Teacher Certification.
The following suggestionsmay serve as evidence and are for your consideration only. The applicant does NOT have to have all of the evidence listed and may be able to identify other evidence not included in this document that supports his/her professional responsibilities.
THE APPLICANT MUST SUPPLY THE EVIDENCE. IF HE/SHE IS UNABLE TO PROVIDE THE EVIDENCE, THE SYSTEM MUST CONSIDER WITHHOLDING VERIFICATION SIGNATURES AND REFUSING TO SUBMIT THE APPLICATION UNTIL SUCH TIME AS THE APPLICANT CAN PROVIDE EVIDENCE OF PRACTICING PROFESSIONAL RESPONSIBILITIES. THE STATE REVIEW TEAM WILL NOT CONSIDER AN APPLICATION WITH MISSING APPROPRIATE SIGNATURES OR CHECKMARKS.
NOTE: This checklist and the applicant’s evidence are not uploaded with the applicant’s other documents. It should be used as a guide and then kept on file with the evidence provided by the applicant.
The Local System Review Team should ask: “Does the evidence provided by the applicant demonstrate pervasive and consistent practice by the applicant overtime?”
