ETFO CAUCUS MEETING APPLICATION FORM
Purpose:
To provide an opportunity at the Annual Meeting for members to hold and/or participate in self-directed caucus groups scheduled for Monday, August 13, 2018, from 10:00 a.m. to 3:00 p.m. Location of each caucus room to be determined and will be posted at the registration desk. The formal portion of the Annual Meeting commences at 4:00 p.m. in the Grand Ballroom.
It is the Federation’s intent that this portion of the Annual Meeting be by members, for members.
To request a caucus, please submit this completed form via e-mail no later thanApril 30, 2018 approved, a meeting room will be assigned (subject to hotel space availability).
A light lunch for participants is provided. The caucus schedule will be posted on the website with the Annual Meeting agenda ( About ETFO Annual Meeting 2018).

As caucus meetings are member driven and not part of the business of the Annual Meeting, please note that if you choose an earlier time slot, accommodation will not be provided for the Sunday night. It is your responsibility to arrive earlier on Monday morning. If your room is not ready, monitored luggage storage will be provided.
