ZERO TOLERANCE

The NHS operate a zero tolerance policy with regard to violence and abuse, the practice has the right to remove violent patients from the list with Immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety.

If a patient is violent or abusive they will be warned to stop their behaviour. If they persist we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.

DATA PROTECTION

All information held about patients is completely confidential. The Practice is registered under the Data Protection Act 1984. This Act protects data held on the computer system.

111 is a new service that has beenintroduced to make it easier for you to access local NHS healthcare services. You cancall 111when you need medical help fast but it’s not a 999 emergency. NHS 111 is a fast and easy way to get the right help, whatever the time. Accepting calls from 18.30-08.00 daily, weekends and Bank Holidays (This number is free to call.

NAMED GP

In accordance with the current government and our NHS contract all patients who are on the practice list prior to 1st April 2015 will be given a named accountable GP by 30th June 2015. Newly registered patients after 1st April 2015will be allocated a named GP on registration or within21 daysof registration and having their new patient checks.

APPOINTMENTS All afternoon sessions are by pre-booked appointments only, as of October 2016, these appointments can be made in person, online or by telephone. We have specific appointment slots available for on the day bookings as an emergency, only in the mornings. All receptionist will take information regarding the symptoms and cross check with a doctor when needed. If you are unable to attend for your appointment please let us know so that we can offer this to another patient.

HOME VISIT REQUESTS

Patients are requested to telephone before 11.00am if a visit is required that day. Emergency visits only will be arranged after that time. Please give the receptionists as much information as possible to enable the doctor to allocate priority to house calls. Please remember that several patients can be seen at the surgery in the time it takes to do a home visit.

TEST RESULTS

In order to find out about any blood, x-ray or ultrasound results please call after 10.00 am Mon-Fri.

PRESCRIPTIONS

ROUTINE— requests for repeats prescriptions will be dealt with within 48 hours. You could drop in your request to the surgery, fax it to us or request it via your pharmacy,

EMERGENCY APPOINTMENTS

Emergency appointments are not for repeat prescriptions, fitness for work certificates or for signing forms.

COMPLAINTS

The surgery operates its own complaints procedure under NHS regulations. All complaints must be addressed, in the first instance to the Practice Manager. Please ask receptionist for a complaints form. The practice may need to provide information about the patient, and treatment they have received, to insurers or legal advisors. PATIENT’S RIGHTS AND RESPONSIBILITIES

You have the right to access the service we provide according to your clinical needs. You have the right to expect a standard of care that would reasonably be provided by one of the Doctors or Practice Nurses. You have the responsibility to attend appointments punctually and cancel appointments that you are unable to attend. You have the right to see and obtain copies of your own medical records, please be aware there will be a charge.

NHS staff involved in the care and treatment of patients will have access to your medical records. Information will never be disclosed to any other source or third party without the express prior written consent of the patient.

DISABLED ACCESS

Wheelchair access to the building is through the main door. A disabled patient's WC is provided on the ground floor. We also have a lift to access the second floor consultation rooms.

EPS –Electronic Prescription Service

The electronic prescription service enables patients to collect their prescriptions from their allocated pharmacy. The request is made electronically via the online service at the GP surgery and sent over to the pharmacy of your choice to collect.

Please ask reception to give your online log on details to request your medications electronically.

Please note to select your choice of pharmacy before hand so they can set it up for your whilst you are at the desk.

VISION ONLINE

VISION ONLINE allows you to book appointments via the online portal with your GP. You could also cancel appointments and request for repeat medication. Please ask reception for further information.

FREEDOM OF CHOICE

The Freedom of Information Act 2000, obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception.

HOW TO REGISTER AS A PATIENT

If you are new to the area and are requiring to register with one of our GP’s please ask at our Reception. If you have your Medical Card then please bring this along with you. Following your registration you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our Practice Nurse. You can access the registration form from our website, however will be required to sign a clarification of identity when you come in to the surgery.

MINOR SURGERY

Dr Salam carries out minor surgical procedures including cryotherapy for warts and verrucae at the surgery by appointment. The doctors will be happy to advise you on this.

NON-NHS EXAMINATIONS

The doctors are happy to carry out medicals, for insurance purposes, HGV and taxi cab licences, by appointment after surgery hours. Please telephone the surgery for an appointment. Please ask the reception staff about the charges for these services.

CLINICS

Chronic disease management clinics run by our practice nurse ( by appointment only) – asthma, chronic obstructive airways disease, diabetes mellitus, coronary artery disease

6 week postnatal and 6 week baby clinics:

These are run weekly by Dr Kana and Dr Salam with the assistance of our Health Care Assistant, Joy.

Cervical Smear Test : by appointment with the practice nurse.

Family Planning -Contraceptive care is provided by the doctors during surgery hours. The surgery at present does not offer IUDS or Implants.
