Christopher Mays, Executive Director. 207.697.2020,

This is where we consider your corporate structure so we can direct you to questionnairesections appropriate to what your organization. This time you will likely have only one “yes” item.

Section 3: Governance

In this section you tell us the legal framework for governance of your organization or service. We realize that many of the facilities we deal with have a complex structure, for example where one entity may own the land, another may provide management services, and a third actually operates the service. In those situations, we invite a separate response from each. If you are reporting on a direct service provider (“A number” = 1 or 2) then it is the operating entity (corporation, partnership or other) that actually is the operating entity what you are telling us about.

In this section we want you to pick just the one response that best describes your situation or comes closest to doing so. If you choose ”Yes, this comes close to an accurate characterization”to one of ourquestions, please use the space given or attach a separate page to describe your situation more precisely.

Remember:choose only onepositive response in this section.
