The following terms and conditions apply to all students currently enrolled in the Engineering Science (ENSC) Program at SFU which includes a mandatory co-operative education component. The information on this form is collected under the general authority of the University Act (R.S.B.C. 1979, c.419). It is related directly to and needed by the Engineering Science Co-operative Education Program in order to facilitate Co-op services. This agreement is consistent with accreditation standards outlined by the Canadian Association for Co-operative Education (CAFCE).

Please read this document and initial each of the points below. I agree that:

  1. I will complete the required number of work terms and work term requirements to meet my degree requirements and will follow the suggested schedule but understand that I may need to alter my semester course planning to accommodate my work term requirements. If I plan to complete more than the required 3 work terms, I will first discuss my plans with a co-op coordinator. If I plan to apply for a co-op work term waiver, I will do so before the last day of my first co-op seeking term.

  1. I will follow guidelines established by the ENSC Co-op Program including attending required sessions and workshops, following of application procedures including use of the online job posting system, interview processes, acceptance of positions, and completion of required forms. I understand that I am required to complete an oral presentation, a technical work report, and an executive summary during my co-op career in order to pass my work terms and complete my graduation requirements.

  1. I will provide accurate and appropriate information regarding my qualifications to the ENSC Co-op Office and prospective employers. I will check my email regularly and promptly respond to all messages from the Co-op staff.

  1. I consent to the disclosure of my academic transcript, resume, cover letter, and any portfolio pieces to current and potential employers in order to facilitate the co-op hiring process. I understand that my consent to disclose the personal information about me noted above expires when I leave the ENSC Program.

  1. I understand that if I wish to take a course concurrently with my work term, I must seek approval in writing from my hiring manager and approval from an ENSC co-op coordinator.

  1. I will keep co-op job posting information, employer lists, contacts, passwords to website, etc. confidential and not share them with anyone.

  1. I understand that the ENSC Co-op Program does not guarantee work placements before or after graduation. I will also conduct my own co-op job search if necessary and will obtain approval from my coordinator if I am interested in pursuing a self-directed work term. I agree to NOT solicit positions from SFU co-op employers.

  1. I will pay my work term fee by the course registration deadline set by the University. If applicable, I will pay the necessary expenses if travel or relocation is necessary and is not subsidized by the company. If my practicum is extended for an additional semester, or if I return to work for an additional work term with a previous SFU co-op employer, I will register and pay tuition and applicable student fees.

  1. I will inform an ENSC co-op coordinator if there are any issues, changes, events, or problems related with my involvement in the ENSC Co-op Program or on a work term.

  1. I will end my academic career with SFU ENSC on an academic term and not a co-op work term.

  1. I understand that if any of the following occur during a co-op work term I will receive a failing grade:
  2. receive a less than satisfactory employer evaluation
  3. breach the employer contract agreement(s) e.g. Non-disclosure Agreement
  4. behave inappropriately (according to SFU Policies) for the duration of the time I am participating in the ENSCProgram

initial / 12.*** FOR INTERNATIONAL STUDENTS ONLY: I understand that before accepting a co-op work placement in Canada, it is my responsibility to obtain and submit copies to the co-op office of the following documents:
A. Valid Co-operative Education work permit through Citizenship & Immigration Canada
B. British Columbia Medical Services Plan (MSP) coverage.

I understand the failure to comply with these terms and conditions may result in an inability to obtaina work term or a non-passing grade on a work term. SFU Engineering Science Co-op reserves the right to dismiss students for improper behaviour at the discretion of the Coordinator(s). SFU Engineering Science reserves the right to update, modify, or revise these terms & conditions.
