Boehringer Ingelheim and BIPI/BIUSA Wellness care about your health and well-being, and have implemented an important program to encourage you to pursue or maintain a regular exerciseprogram. This offering provides you with access to a reimbursement when you join a commercial or community fitness club or fitness center. Those eligible will receive an annual fitness club membership reimbursement for up to a maximum of $150 per year. (If your membership is less than $150, you are eligible to receive the amount of the membership). In order to qualify for the reimbursement, the facility you join must meet all of the qualifications listed. If your club meets these criteria, simply complete and submit aFitness Center Reimbursement Program Request form!

BIPI Field employees
Southbury employees
Corporate Center employees (members of matrix Fitness Center only)
Employees who are members of the onsite facility located on the BIPI main campus in the AOB are not eligible as this fitness center is already subsidized.
FitnessCenter Qualifications:
This facility currently:
? Has at least one staff member who holds a current CPR certification on site at all times.
? Requires staff responsible for fitness programming to have at least a B.S. degree in exercise science or related field or hold a current certification from one of the following internationally recognized associations (ACSM, NASM, AFAA, ACE).
? Has at least 3 different pieces of cardiovascular equipment, (ex. bicycles, stair climbers, treadmills, rowers).
? Has strength training equipment that targets all of the major muscle groups, (biceps, triceps, chest, back, shoulder, hamstrings, and quadriceps).
? Has all new club members fill out a pre-exercise screening questionnaire, and recommends that members with risk factors obtain physician’s consent before starting an exercise program.
? Has clearly posted emergency procedures, and an easily accessible and fully stocked first aid kit.
? Makes membership available to persons of all races, creeds and national origin.
? Does not sell prepaid lifetime memberships.
? Provides new members with information regarding:
? Terms of membership.
? Rights to cancel membership.
? Refunds to members who relocate; or are unable to engage in fitness activities due to medical reasons that can be documented.

The qualification form must be filled out by the participant and the fitness center manager.
