Why East Lancs?

The East Lancashire Hospitals NHS Trust andPrimary Care Trusts provide services to the population of East Lancashire of about 510,000 people. Geographically it covers the Blackburn, Burnley, Hyndburn, Pendle, Ribble Valley and Rossendale areas.

The posts are based in the Royal Blackburn Hospital and Burnley General Hospital. The Trust offers a comprehensive range of “in” and “out-patient” services, including psychiatryand outpatient clinics from a range of visiting specialities by tertiary referral. There are full diagnostic and scanning services available. Both sites are also fully supported by Postgraduate Education facilities, fully equipped libraries and subsidised staff gyms.

We have also found that 88% of East Lancashire Foundation Trainees have achieved their first choice of Specialty Training posts!!!

The Trust has undergone a major new review involving relocation of services between the two hospitals and now includes The Lancashire Women and Newborn Centre which is the culmination of more than five years’ work and brings together all of East Lancashire’s Women’s and Newborn services, enabling the delivery of the best possible care. The state-of-the-art building located on the Burnley General Hospital site includes:

• A Central Birth Suite providing hi-tech facilities for complex births
• An adjoining midwife-led Birth Centre which offers a relaxed environment where healthy women can go through labour naturally and at their own pace in comfortable rooms with en-suite facilities
• A Neonatal Intensive Care Unit caring for premature newborns from across North and East Lancashire and providing high level care including whole-body cooling treatments for the most poorly babies
• A purpose-built Gynaecology Unit incorporating separate areas for services such as oncology, urogynaecology, fertility treatment and termination of pregnancy

Royal Blackburn Hospital

This new build hospital completed in 2006is located approximately 1 mile outside of the town centre. There is a regular bus service which operates past the main entrance. Acute services are based at this site including emergency medicine, surgical emergencies, high risk surgery and a helipad. There is anextensive dining room serving breakfast, lunch and evening mealwhere specialist diets and out of hours are catered for. There is also a newsagents,café and subsidised staff gym. In the Learning Centre there is a fully equipped library, seminar rooms and a large auditorium all fitted with visualising equipment. There is also a large clinical skills lab with kitchen and a 3G simulation room, with a new state of the art doll which can be programmed to simulate clinical emergencies.

Burnley General Hospital

Thislarge district hospital is situated 2 miles from the town centre and a regular bus service operates past the main entrance. Since the mid 1960’s there has been extensive development and refurbishment – including the new main entrance ‘Phase Five.’ Current developments are now complete and Elective Surgery, Maternity, Gynaecology and the Neonatal Unit is now on this site. There is also a Psychiatry Department which specialises in Child & Adolescent Psychiatry and an Urgent Care Centre for minor injuries. As a Foundation Trainee you will be expected to work on both sites as your rotation requires you to.

Teaching

Teaching is held weekly, FY1 – Tuesdays 12.30–1.30 pm (Blackburn & Burnley sites) Thursdays 2-5 pm (Blackburn site) and FY2 – Tuesdays 2-5pm (Blackburn site). Most teaching is in small groups and is structured to be as interactive as possible. The topics are split between patient safety, careers and specialities. The teaching evaluation is monitored closely and feedback is expected each week. There are also allocated Foundation Forums in the teaching programmes,bi-monthly, to encourage feedback.

Opportunities Available

As part of your Foundation Year 1 you will be expected to have completed Immediate Life Support a one-day course accredited by the Resuscitation Council (UK) and ALERT (acute life threatening events recognition & treatment).

As part of Foundation Year Two you will be expected to complete Advanced Life Support a one-day course accredited by the Resuscitation Council (UK).

Shadowing

Before new Foundation Trainees start with the Trust we hold a shadowing period which you will have a chance to be involved with and have your own trainee shadow you in your work.

Taster Sessions

If you have not had chance during your rotations to gain experience in a department you are interested in, a week’s taster session can be arranged so you gain this valuable experience and broaden your knowledge.

For all courses you will have to apply for study leave and will be supported by the Medical & Dental Education Department.

Job Descriptions

Integrated Medicine

The department of Medicine has a full team of on-site Consultant Physicians covering General Medicine, Endocrinology, Gastroenterology, Elderly Medicine, Cardiology, Respiratory Medicine, Rheumatology, Dermatology, and Genito-urinary Medicine.

You will gain experience in both an in-patient and out-patient setting with appropriate exposure to emergencies and will be part of a team including Consultants and Specialty Trainees. The FY2 posts are currently attached to General, Rehabilitation and Stroke Medicine. You will be expected to attend, and participate in, educational activities including grand rounds and audit presentations. The FY2 rotas are full shift and all are compliant with the EWTD.

The competencies attained in Integrated Medicine will include:

~History taking

~Examination and selection of appropriate investigations

~Clinical skills e.g, lumbar puncture, chest and peritoneal drains and central lines

~Acute assessment of medical problems

~Co-ordinating treatment and investigations

~On-going management of patients including liaison with other teams and discharge planning

Genito-Urinary Medicine / Histopathology / Clinical Microbiology / Infection Control

This innovative FY1 post will involve work both in the GU Medicine Department, Infection Control, Clinical Microbiology and the Pathology Department. This post will be un-banded.

The learning objectives for these departments will include:

Microbiology:

  • Learn about the role of the microbiologist in dealing with patient referrals, e.g. the weekly blood culture results, by shadowing the microbiologist on call, reviewing patient notes and then discussing cases with consultants in form of mini-CEX/CbD
  • Understand the treatment and prevention of C.Dificile and MRSA by attending C.Dificille ward rounds
  • Extending personal knowledge of infection control by spending a day with the infection control nurses/team
  • Understand the role of the microbiologists in various settings within the hospital by attending ICU ward rounds to review complex patients and the appropriateness of antibiotic treatment
  • Review the role of various health agencies such as HPA and their role in communicable diseases by arranging a day with the local HPA team
  • Develop the skills needed to analyse samples within the lab by spending a day in the laboratory with staff shadowing and reviewing culture plates with staff

Pathology/Histopathy:

  • Learn basic skills in analysing histological samples and become competent in using a microscope by attending sessions with the histopathology team
  • Understand the role of pathologists/histopathologists within an MDT setting by attending any MDT meetings with the histopathology staff and complete a reflective piece of work
  • Attend a post mortem / autopsy, and understand the role of the pathologist regarding post mortem examinations

Blood Sciences:

  • Understand the techniques used to analyse and authorise blood science results by attending lab sessions with technicians and review blood results with blood science consultants
  • Understand how the transfusion system operates by attending the transfusion department and review notes obtained following transfusion assessments

GU Medicine:

  • Learn how to take a focused sexual health clinic and perform appropriate examinations by observing and performing observed history taking and examinations during the weekly GUM clinics
  • Develop own knowledge of the treatment of common GUM conditions by producing a short presentation and quiz to fellow foundation year colleagues
  • Understand the importance of confidentiality and the measures taken in the GUM setting by discussing with staff members confidentiality and methods used, and any circumstances where it can be broken
  • Understand the role of health practitioners in contract tracing, and the various methods used by observing contract tracing being completed, and produce a reflective piece on the advantages and disadvantages of each
  • Develop skills in HIV pre-test counselling by observing pre-counselling in action, and complete an assessment with staff
  • Understanding the role of the community teams within the GUM specialityby attending family planning clinics at Larkhill, and conducting a questionnaire to help develop services available for patients

Learning Objectives

  • Complete an audit, and seek out presentation opportunities where possible. When working in the various departments inquire as to possible audits already started, or being planned with a view to complete/help

Psychiatry (Lancashire Care Trust)

The Psychiatry element of the FY1 and FY2 tracks will be fully integrated in the duties and working patterns of colleagues in the Lancashire Care NHS Trust. All duties are worked on a full shift system which includes out of hours. The Trust covers a number of sites and provides psychiatric services to in and out patients, and in addition to working in specialist units you will carry out assessments in acute settings and in the community, working with General Practitioners. There is a full supporting Consultant and Middle Grade team, together with hospital and community based specialist nurses and support workers.

The competencies attained in Psychiatry will include:

  • Assessment and management of psychiatric illness
  • Management of acute illness (including acute confusional state and acute psychosis)
  • Prioritisation of work-load
  • Discharge planning
  • Risk assessment
  • Understanding the life style effects on health from alcohol and drug usage

Paediatrics and Neonates

There are Foundation Year posts available in Paediatrics (FY1) and Neonatology (FY2). The department offers a wide experience over a range of Paediatric interests and includes exposure to NICU, paediatric medicine and paediatric sub specialties, including out patients.

There is full Consultant, Specialty Trainee and Specialist Nurse support at all times to all trainees. The FY1 posts are based in Paediatrics and are un-banded. The FY2 rotas are full shift and compliant with the EWTD. With the opening of the new Women’s and Newborn unit FY2 posts will be based in the Neonatal Unit in Burnley.

The competencies attained in Paediatrics and Neonates will include:

  • Management and care of the acutely unwell child
  • Resuscitation of the new-born and children
  • Drug-dosing
  • Cannulation and airway management of the new-born and children
  • Vaccination policies
  • Communicating with children and parents

Obstetrics and Gynaecology

The department delivers care to over 6,000 women per year. There are 13 Consultants working within the department whose special interests cover maternal medicine, foetal medicine, minimal access surgery, infertility, gynaecological oncology, ultrasound, uro-gynaecology, colposcopy, early pregnancy problems and medical education. You will be part of a team, including Consultants Specialty Trainees and you will be based at the new unit at Burnley. The posts are available in FY1 (un-banded) and FY2 which are full shift and compliant with the EWTD.

The competencies attained in Obstetrics & Gynaecology will include:

  • Communication skills
  • Breaking bad news
  • Care of the pregnant women
  • Management of acute emergencies, especially haemorrhagic shock
  • Management of early pregnancy problems

There is a strong educational programme within the departments.

Surgery

The department covers general surgery, vascular surgery and urology. There are a full range of sub-specialities within the department. You will be part of a team, including Consultants and Specialty Trainees. The posts are available in FY1 and FY2. The FY1 posts include a one-week placement in Critical Care. The FY2 rotas are full shift and are compliant with the EWTD.
