The attached form is designed to assist you in setting up safely for the work you are about to undertake, identify the hazards associated with the work and assess the most suitable and safest way of completing the work.
You should always Eliminate (E) a hazard if you can. Where this is not possible you should Isolate (I) a hazard and if you cannot do either you must Minimise (M) the hazard. There may be occasions where multiple controls are appropriate.
Use this checklist, to identify hazard/s. Decide the best option/s available to control the hazard/s for the task you are about to do, brief everyone doing the work, and ensure the appropriate controls are in place. Ensure you sign the form and keep it as part of your job documentation.

Earthquake Related Safety Check List

It is absolutely imperative for both yours and the customer’s safety that you complete the following inspections and checks prior to starting your work, particularly in homes constructed with brick. Work must not proceed if you decide that any hazards or damage found compromise the health and safety of you or any person in or within the vicinity of the work place.

Hazard Identification & Management (E, I, & M)

Work through the listed hazards in the left column and if they are present for the work being undertaken, choose and highlight which Control/s you will use to Eliminate, Isolate or Minimise these hazards.
