Teacher Licensure Level EESLPD Mentor

In compliance with NC State Board of Education Policy, all NC Lateral Entry (LE) licensed teachers must complete a 10-day pre-service program. Additionally, all NC LE license teachers must complete a minimum of 6 semester hours annually according to their plan of study from an accredited higher education institution to meet licensure requirements.

Your EESLPD assigned mentor will assist you in completing this checklist. Be sure to maintain a professional file with copies of all your documentation. After you have successfully completed the items on the checklist, you will share the documentation with your mentor. This completed checklist will be given to your mentor who will mail it to the EESLPD office.

We value your opinion and constantly look for ways to improve. In May, you will be asked to complete an on-line survey to share your experience. Please make sure you have a functional email address to ensure you can participate in this critical process. Use the Change Form ( to notify the EESLPD Unit of any changes.

EESLPD Orientation and NC Teacher Evaluation Process Professional Development(PD)–provided by the EESLPD offices. PD includes State Board goals, licensure process, teacher evaluation, mentoring, developing a Professional Development Plan (PDP), Code of Professional Practice and Conduct for NC Educators.
Complete Self-Assessment using the Rubric for Evaluating North Carolina Teachers located in the
North Carolina Educator Effectiveness System (NCEES) ( after you have completed the training. Self-Assessment should be completed before the evaluator completes the first observation using the Rubric for Evaluating North Carolina Teachers by October 31st.
Complete NC FoundationsAdvancedOnline Training Modules 1-6 according to the NC Foundations Pacing Guide and attach the Foundations Tracking Sheet completed by the EESLPD mentor and /or evaluator as evidence of completion of all module activities including preand post learningand Instructional Practice Checklists for Teachers (modules 2-6).
During classroom visits, the evaluator will review and document that lesson plans are aligned with NC Foundations for Early Learning and Development.

Guidelines for Completing the Pre-Service Checklist

Teachers with NC Lateral Entry Provisional Birth-through-Kindergarten License

EESLPD Orientation and Training
Provided by the EESLPD offices. These sessions will include information about State Board goals, the licensure process, teacher evaluation, mentoring, developing a Professional Development Plan (PDP), Code of Professional Practice and Conduct for NC Educators, etc. Include your training certificate in your professional file as documentation.
Lateral Entry Teacher Self Assessment
Before the evaluator conducts an observation, the mentor will assist you with accessing NCEES/Home Base where you will compete the Self Assessment to reflect on your classroom and teaching practices. This self-assessment will be used to develop your Professional Development Plan (PDP). You will also print a copy of your Self Assessment and include it in your professional file as documentation.
Foundations, Early Learning Standards
Complete Foundations Online Training Modules 1-6 according to the NC Foundations Pacing Guide and attach Foundations Tracking Sheet as verification with this log.As you complete the online modules both mentors and evaluators will follow-up, as outlined in the Pacing Guide, and support teachers in implementing new strategies in their teaching practices.
Letter to Family Introducing Yourself
Provide a copy of the letter you shared with families introducing yourself.
Documentation and Reflective writing from Guided Observation at a Demonstration Classroom
The EESLPD Mentor will arrange for you to visit a demonstration classroom approved by the EESLPD. Please provide a date that you are scheduled to attend a guided observation at one of the EESLPD Demonstration Sites.
Once that observation is completed, no later than January 31st of the current school year, use LE-Handouts #6a and #6b to prepare a reflective report about your visit. Share this report with your EESLPD mentor to complete this checklist.
