The purpose of this challenge is to encourage and reward best practices of the sponsors of the Summer Food Service Program (SFSP), also called Summer BreakSpot.
Each category has a Bronze, Silver and Gold level award.
To Apply
· Fill out the general application and indicate which award(s) you are applying for. There is no limit to the number of awards you may apply for.
· Specify each award category you are applying for and submit any supporting documentation that will validate your entry(s). Sponsors may use multiple sites to validate their qualifications.
· Sponsors should include supporting documents to show their qualifications for the award(s).
· The application, summary and supporting documentation should be submitted to Rachel Mohler at .
· The application must be received by October 2, 2015, by 5:00 p.m., Eastern Standard Time.
Award Process
· The Division of Food, Nutrition and Wellness (FNW) will notify the sponsor by email when the award application is received.
· FNW will review the sponsor’s award application and may request additional documentation from the sponsor.
· An awards committee will determine the winners based on validation and documentation of the award applicant.
Awards Announcement
· FNW will announce winning sponsors at the 2015 Fall Statewide Sponsor Meeting.
· Winners will be recognized on FNW’s website.
· FNW will notify the United States Department of Agriculture Southeast Regional Office of the award winners for possible regional and/or national recognition.
Prize
· The winning sponsors will receive a 2015 Summer BreakSpot Challenge plaque.

Rising Star Award
The Rising Star Award is presented to a new SFSP sponsor that ran a superior first year of operation. This sponsor will have received excellent reviews from the FNW Accountability Team and the Florida Department of Health. The sponsor will have had an outreach program for their sponsorship as well.
Provide a detailed summary of your program, including notes regarding state monitor reviews and Department of Health reviews, as well as any outreach, special events or activities that were affiliated with your program.
Increased Expansion and Participation Award
This award category is for the sponsor that showed the greatest expansion between 2014 and 2015. In addition, this sponsor will have serviced sites that were brand new to the SFSP.
Provide a detailed summary of your sponsorship’s efforts that increased expansion and participation in the SFSP. Documentation should include meal count data showing the number of meals served, the number of sites serviced under your sponsorship and any other supporting documentation.
Innovation in Operation Award
This award will be presented to the sponsor that demonstrated innovative ideas when implementing its Summer BreakSpot. Innovative ideas could include mobile feeding, adding new sites to underserved or rural areas and/or activities that promote consistent participation.
Provide a detailed summary on the ways in which your sponsorship was innovative in the way it operated. Documentation may include, but is not limited to, photos, community statements, activities and outreach ideas.
Farm Fresh Healthy Meals Award
This award category is for the sponsor that provided meals that not only met the meal pattern guidelines, but were creative, nutritious and kid-friendly. Along with serving healthy meals, this sponsor will have also provided a generous helping of fresh, local Florida produce. As a bonus, show us how you incorporated nutrition education into your meal service to make trying new foods fun for children.
Provide a detailed summary of how your sponsorship provided farm fresh healthy meals to the community that you serve. Supporting documentation may include, but is not limited to, menus, recipes, bidding information/invoices, photos, community statements and/or items used to provide nutrition education in the meal service.
Above and Beyond Award
This award is targeted for the sponsor that excelled in all of the categories listed. Did you meet the criteria for increased expansion and participation, have innovative ideas for your sites and serve healthy, farm fresh meals? If so, we want to hear from you!
Provide a detailed summary telling us how your sponsorship went above and beyond with summer feeding. Remember to include documentation showing how you increased expansion and participation, implemented innovative ideas and served healthy, farm fresh meals.
Faith-Based Commendation
Faith-based organizations play a vital role in the communities that the SFSP serves. This award is for the faith-based sponsor that took on the challenge of summer feeding.
Provide a detailed summary explaining how your faith-based organization accepted the challenge of participating in the SFSP as a sponsor and provided services to the communities in which you serve.
