Computer Check-out Form

DSCC policy for removal of Non-Media Equipment Off Campus:

  1. Request for equipment check out should be made to the supervisor responsible for the Inventory of the Item.
  2. No Items under maintenance contract will be allowed under the check out policy (e.g., typewriters, office equipment, etc.) unless approved by the Business Manager.
  3. AV equipment and media may be checked out from the Library in accordance with present practice.
  4. “Check out” does not include the removal for personal use of expendables (toner or ribbons for printers, lumber, etc.).
  5. Items routinely removed from the campus for extended periods by virtue of position or activity shall be allowed without repeated checkouts following a proper notification to the Vice President for Technology that these Items are to be removed from campus on this basis (e.g., cameras, microcomputers, etc.).

DSCC computer check-out policy:

  1. Equipment is available for checkout for the purpose of performing work-related activities atthe home of the employee in the event of a pandemic or other emergency. (Classified staff whoare requested to work overtime at home by their supervisor must be paid for their time.) If the Microsoft Office suite is desired, the employee can pay a $9.95 fee to Microsoft for the Microsoft Office Suite.
  2. Since this equipment is being provided for the employee’s use in the event of a pandemic event, the employee is required to obtain and retain some form of internet access to be used with this computer.
  3. It is the responsibility of the employee to purchase and install any equipment and/or software necessary for internet access.
  4. The computer will be loaded with an operating systemand MicrosoftForefront Security Suite for anti-virus protection. Microsoft Forefront Security Suite will be installed on the PC. Both the operating system and Forefront will be set to automatically update. It is the responsibility of the employee to verify that timely updates for the operating system and Forefront security suite are applied.
  5. Only one copy of the Microsoft Office software obtained through the Home Use Program may be installed on a computer at the employee’s house.
  6. A 17-inch monitor, keyboard, mouse, two power cables, video adapter, and external power adapter for the computer will be supplied with the computer. The employee must provide a surge suppressor for this equipment to be plugged into.
  7. If an employee has any problems with the computer equipment requiring the assistance of Computer Services, the equipment should be returned to Computer Services for replacement. No data from any returned computer will be saved.
  8. When an employee that has checked out a PC leaves DSCC employment, the PC must be returned to DSCC. If the employee has a copy of Microsoft Office owned by DSCC loaded on a personal PC, it must be deleted. Failure to return the equipment will result in a charge of $1200 to the employee.
