I. PROPOSAL SUMMARY― one-half page, maximum
Please summarize in a short paragraph the purpose of your organization. Briefly explain why your organization is requesting this grant, what outcomes you hope to achieve, and how you will spend the funds if a grant is made.
(Sections II through III not to exceed four pages)
II. FUNDING REQUEST
1. Please explain the project including:
- A statement of its primary purpose and the need or problem your project will address
- The key strategies you will employ to implement your project and length of the project
- The proposed staffing pattern for the project and the names and titles of who will direct the project
- Explain how a SPF grant will assist in bringing the project to its next level of impact/effectiveness
- Describe how your project aligns with Sustainable Path’s focus on science and whole systems thinking
- Briefly describe your organization’s relationships – both formal and informal – with other organizations working to meet the same needs. Explain how you differ from these other organizations.
III. EVALUATION
Explain how you will measure the effectiveness of your activities. Describe your criteria for a successful program and the results you expect to have achieved by the end of the grant period.
IV. ATTACHMENTS
- A current expense budget for the project. Indicate the specific uses of the requested grant, if possible.
- Aligned side by side on the same page, your operating expense budgets for the current and previous fiscal year
- Please list the foundations, corporations and other sources that you are soliciting for funding, and the status of your proposal with each (to the best of your knowledge).
