(a)Meters User Block Schedule:

(b) Metered User Minimum Schedule:

Each user shall pay a minimum charge in accordance with the following applicable size of meter

installed, for which the user will be entitled to the quantity of water set out in the above schedule of

rates.

(d) Connection (Tap) Charge:

Each user at the time he is connected with the water works system shall pay a charge to cover the costs of:

excavating and tapping the main; furnishing and installing service pipe from the main to the lot line; furnishing and installing corporation and stop cocks; and furnishing and installing meter crock (if outside), yoke, and meter. The charge for a 5/8 inch meter tap shall be $750.00. The charge for a

larger meter than the 5/8 inch meter tap shall be the cost of labor, materials, power machinery,

transportation, and overhead incurred for installing the tap, but shall not be less than the charge for a

5/8 inch meter tap.

(e) Reconnection Charges or Service Fees:

When the Utility serves notices of nonpayment and proposed disconnection upon the customer and an employee of nonpayment and proposed disconnection upon the customer and an employee of the waterworks appears at the customer’s location at the time specified in the notice for disconnection, a

re-establishment of service is required by any one customer or continuation of service is required by

the customer a reconnect fee or a service fee will be charged. This charge will be made by the

waterworks tocover the cost of potential discontinuance, re-establishment or continuance of service.

The charge with any arrears due the waterworks, shall be paid by the customer before service will be

re-established or continued.

When the service is turned off for reason other than nonpayment but for reason beyond the control

of the Utility, and a re-establishment of service is required by any one customer, this charge will be

made by the waterworks to cover the cost of discontinuance and re-establishment of service. The

charge, together with any arrears due the waterworks, shall be paid by customer before service will be

re-established.

(f) All bills for water service not paid within seventeen (17) days after the bill is mailed shall be subject to a late

payment charge of ten (10) percent of the first three (3) dollars and three (3) percent of the excess

of three (3) dollars.

(g)Bad Check Charge:$25.00

Any customer or water user of the Dubois Water Utilities, Inc., upon payment of their account, submits

a bad check shall be assessed a $25.00 charge.

(h) Membership Fee:$100.00

A membership fee of $100.00 for all property owners shall be paid in addition to the above charges for connection fees, upon application for membership in the Corporation. The Membership is non-

refundable and shall be transferable only upon the books of the Corporation’s Secretary.

(i) Membership Transfer Fee:

Transfer of membership on the records of the Utility, including the issuance of a new membership certificate

and transfer of all billing records. $20.00

(j)Member Selling Membership - Administrative Fee:

Transfer of Membership that is being sold to a new Member; the member selling the Membership will

be charged a fee for the administrative costs involved in the transfer on the waterworks records.

(k)Name (billing) Transfer Fee:

Transfer of name/address on the records of the Utility for that person who is responsible for payment

of bill and to whom monthly statement is to be sent.

(l)Special Meter Reading Request Fee:

Transfer of Membership and/or transfer of name/address as above, but with addition that member or customer has requested a special reading of water meter on a specific date (non-regular) date.
